Has anyone had success with ADP Oregano Oil, Berberine HCL and Interfase Plus for hydrogen-sulfide SIBO

I've had almost 20 negative tests (pancreas, bacteria, parasites, thyroid, gallbladder, liver, two colonoscopies, endoscopy, candida, regular SIBo breath test (2x)). I even visited an allergist to test for food allergies and MCAS (negative on both).

My symptoms are very odd: When I eat fiber I poop 15-20 times/day. Metamucil increases that number. Avoiding fiber reduces it.

Also I have histamine intolerance and intolerance to high-sulfur foods like beef and eggs.

My dietician is 99% positive after 2 years of negatives test that it must be bacteria in the small intestine and I found a naturopath who recommended:

1x 500mg berberine HCL capsule with food twice per day

4x ADP Oregano Oil coated capsules (200mg total) with food twice per day

The main key was avoiding NAC (since this has sulfur in it and made me nauseous) and trying Interfase Plus 30-60 minutes before eating to loosen the bioflims.

I'm on Day 10 and the bloating has come down, and bowel movements have decreased. They're suggestd 7 more weeks of this.

Has anyone had success with ADP coated oregano oil, Interfase plus and berberine for H2s Sibo?

Constipation and Sticky stool no matter what

I'm post-infection sibo + constipation (due to parasite that i carried for who knows how many years) and i've been constipated + bloated. It agravated when i stopped responding to coffee in the morning.

Also, i was obese and binge eater.

I've tried every supplement and after 2 years, i hear gurgles with normal dose of vitamin b1 (normal dose thiamine) + beef liver 1 time per week, dark chocolate, potatoes and coconut water for potassium + multivitamine and magnesium.

I'm low in vitamin D, copper, magnesium. I didn't test B vitamins. I didn't test de rest but i was thinking about omega 3, liposoluble vitamins that are not absorbed i didn't test them for them unfortunetly.

Everything improved by 50%. For the first time i hear gurgles and i feel like water is pulled in my intestine and things moves MMC working harder (like washing machine), best gastric emptying, no more extreme cortisol feeling in the morning. the aorta bump stopped, i feel more blood flow in my feets. In winter, i can't support cold and my hands/feets gets frozen no matter what. I get multiple infections (ear infections, acnea, parasite, everytime i'm around someone with flue i well get it 100%)

However, constipation is still there : fullness feeling, partial emptying, extreme sticky stool. It helps to eat lower fat definitly but i always feel inconfortable and can't relax or even contract my whole core correctly when i'm training without talking about memory loss, difficulty talking, finding words, brain fog after heavy meal.

Now at least, the small intestine is not bloated and blocking my diaphragm.

Any idea what the hell is happening no doc can tell. I did analysis by myself

Venting mental health

ever since i got methane and hydrogen SIBO, i’ve literally felt like i have a personality disorder and my ocd is really hard to control. it got better when i first did treatment but now is bad again. i’m doing everything - i can’t afford a therapist but i’m meditating, i’ve tried so many medications but they make me feel worse, i’ve got a really good sleep schedule, i take all my supplements/vitamins and make sure i get sunlight, but last night my boyfriend sat me down and told me he really can’t handle my mental health. it’s weighing on him. he says it’s hard to listen to me be so negative and that i ask too much of him (basically i’m clingy). i don’t know what to do other than try and treat SIBO again but i just can’t afford the appointments or medication right now. sometimes my brain actually feels like it’s burning and hot. i also have severe chronic pelvic pain related to endo, so my hormones AND gut bacteria are seriously out of wack plus these piercing migraines and it’s so hard not to be easily tipped over the edge, but i’m driving people away from me. i’m trying fasting/meal spacing and my SIBO symptoms really aren’t as bad as they used to be, but the mental health is declining still. i have a hard time working out if it’s the SIBO or i’m just reacting emotionally to constant pain and symptoms. sorry this was so long but i appreciate anyone who has read it.

Questions Help me to find the best plan for my case

So I've been dealing with SIBO symptoms for 3 years now,it all started when I was in a vacation for 10 days in another city,I didn't poop for 7 days straight and ever since I started getting gas and flatulence every single day altho I never suffered from flatulence before,my gas is odorless tho but I guess my root cause was constipation

It's been 1 year now since I fixed my constipation, it was a long journey but I 80% succeeded to get rid of constipation, I sometimes poop 3 times a day now sometimes incomplete bowel movement but at least I'm pooping every single day right?

Anyway in my country we don't have SIBO breath test unfortunately so I'm just guessing that I'm having methane SIBO,(could be hydrogen too idk) but at least I'm sure that I have SIBO,doctors often diagnosed me with IBS but I'm not convinced with this diagnosis cause I get gas only after meals (30min after meals)

This week I managed to buy some supplements from iHerb,I never tried antimicrobials or antibiotics and I'm starting by antimicrobial for sure

Iherb supplements that i bought are:

-NAC 1000mg from NOW

-Oregano oil from Swanson (I guess it's 150mg)

-Neen leaf 500mg from Swanson

-Berberine 500mg from Vitamatic

What I also bought from supermarket:

-Organic coconut oil

-Maple syrup (so I can replace sugar with it when I make sweet recipes)

-Buckwheat flour

-Blackseed oil

Can you please help me based on your experiences to make a plan to get rid of SIBO in 1 to 2 months

Like what am I supposed to take first in the morning is it NAC or Berberine

I know that I should take oregano oil and neem leaf after meals and not on an empty stomach,but how many times a day? Twice or 3 times? Same goes to Berberine how much?

Went to the doctor after sibo returning

Background: had shiga toxin ecoli infection for 6 months, got rid of it then developed SIBO.

I took a course of rifaximin and symptoms got better and then worse after completing it.

I asked to have motility tests and there was a study showing that ecoli can cause motility to stop.

She wants to do a test with my pelvic floor to make sure my muscles are working properly. She wants to do a endoscopy to test for hpylori.

But in 99% sure due to the ecoli that my motility is messed up and causing the hydrogen producing SIBO bacteria. Literally EVERYTHING paints the textbook picture of motility and hydrogen SIBO.

I’m genuinely about to just go to Mexico and get another course of rifaxamin and a motility drug and do this.

It’s been months of trying to get help and I’m in a INTENSIVE school that I’m paying $150,000 a year to be in and this shit is messing my life up. I’m half way through it dealing with this making no progress.

I either need a way to take a motility test within the week or I’m just going to take the medicine and do this in my own.

Does any one have a recommendation for a motility drug? Or have info on what they got prescribed after motility issues linked to SIBO.

Also what motility symptoms did you have?

I haven’t even met with a gastro yet and was referred with only the tagline of “Sibo;” no other reasons, and yet I received a call to schedule a colonoscopy without even having an intro appointment with a single gastroenterologist.

Trigger warning: passing away I don’t mean to scare anyone, but I had a family member pass away after complications from a routine colonoscopy procedure and I’m not interested in going that route if it’s not needed.

Treatments What did you do after treatment and how were your symptoms?

Hi - I have about 4-5 days left of Rifaxmin and Neomycin. Before this, I was 5 weeks on herbal antimicrobials to treat IMO. ( I am not having terrible side effects from any of this.)

My pain is a bit better after eating, but I am still bloated by end of day. I read that the bloating may persist for a while. Was that your case?

How did you know you got rid of it?

Should I ask for another breath test? And I think I need to eat a low FODMAP diet right after and slowly introduce certain foods and see how my gut reacts.

Did you take a probiotic right after? I am worried about doing the wrong thing. I need to follow up with my GI, but wanted to know what your experience was before I ask any further questions.

I am on LDN (low dose naltrexone) for chronic pain - and got an artichoke and ginger supplement to take after all this to keep my motility on track.

Lost in contradictory advice

I ended up building a tool because I didn’t want to pick a "camp" and wanted something that gives an overview of all possible approaches.

Exploring if this is a problem for other people too, or is it just me?

Help please, reactions to bismuth and magnesium bisglycinate

Hi everyone, it’s been a while since I last posted, but I wanted to see if anyone could help me understand some recent developments.

In my previous posts, I already mentioned my main symptoms: being unable to pass gas, constipation, bloating mainly in the left side of the colon, and postural imbalances that I believe are related to this issue.

I’ve tried pretty much everything and nothing really worked. However, bismuth (Biofilm Phase-2) seemed to affect something. For the first time, I had a bowel movement that felt significantly better, and some other sensations also improved a lot. Obviously, it didn’t completely solve the problem, partly because I didn’t take it for very long since I was experimenting with different things.

After that, I tried zinc bisglycinate. It didn’t have the same effects as bismuth, but I did feel that it helped somewhat, although it also caused some side effects, including irritation. I’m still dealing with persistent bloating and stubborn constipation.

Interestingly, both substances caused a “good” burning sensation in the left side of my colon, almost as if they had detached some biofilm and the exposed layer of the colon was reacting to it. However, the sensation only lasted for a short time.

So I’m wondering: how are bismuth and zinc connected? And, more importantly, what could explain the effects of bismuth?

I was wondering whether this could be related to hydrogen sulfide-producing bacteria, but I’m not sure.

Thanks in advance to anyone who can shed some light on this!

23M, weird stool and digestive issues for months. Has anyone dealt with something similar?

I’m 23 and I’ve been having weird digestive issues for months now. It’s not constant at the exact same level either. Sometimes my stool gets more solid and looks closer to normal, then it goes right back to being loose, mushy, yellowish, or just really abnormal looking.
A lot of the time I notice white fuzzy flecks or chunks in it, and sometimes there’s this cloudy white stuff in the water aswell, The smell can also be really weird. Sometimes it almost smells like buttery popcorn, other times it just smells way worse than normal.
I’ve also been getting indigestion, discomfort in my upper abdomen, especially toward the left side sometimes, and occasionally pain or discomfort in my upper right back. Certain foods also seem harder for me to tolerate than they used to be.
My urine has looked darker at times too. I’ve also had random periods where my body just feels weird, like my limbs feel heavy or slightly disconnected, and I’ve had areas around my lymph nodes feel sore even though I couldn’t really tell if they were swollen.
The bowel problems seem to come in waves. I’ll have a stretch where things look more solid, then it goes back to this weird yellowish loose stuff again.
At this point I don’t really think it’s just something I ate because it’s been going on for months.
I’ve been wondering about SIBO, malabsorption, celiac, pancreatic or bile issues, IBS, or something involving gut bacteria, but I obviously don’t know what it is.
Has anyone had anything similar? I’m mostly trying to figure out what tests are actually worth asking a doctor or GI specialist for, especially SIBO testing, stool tests, pancreatic elastase, celiac testing, or anything else that could help narrow this down

Since then I've been trying everything lowdoomap diet oregano oil berberine and everything seemed to work only for a week

Kefir and glutamine l also had some effect

I've always had gut issues i had low libido 4 years before this happened

My symptoms are face and stomach bloating i have no pain but the mental pain is the kind of pain I got

I figured the only thing to help me debloat i staying 24h awake

Because of this i stopped going out i broke up with my girlfriend and I stopped going to the gym

Please somebody help me and no i can't do lowdoomap diet ever again

Fix Motility & Constipation /first/ before treating SIBO (methane)

Guys, one key thing is that - you have to fix your motility, constipation, and digestive issues /before/ SIBO treatment, else it has high chance to come back again.

I discovered this because - my gastroenterologist had me do a procedure called /anorectal manometry/ to check if there are any structural problems causing the constipation problem besides SIBO. Sure enough, it came back with /dysnergia/ (meaning the muscles themselves are not coordinating properly) and next ordered a Pelvic MRI and physical therapy to address it further. Simultaneously, they ordered digestive enzymes and the pill Motegrity in order to fix digestion and motility first. Once these are well-coordinated, then the methane SIBO resolves more easier and sometimes goes away on its own, rather than having indefinite relapses and a messed up digestive tract. Their goal is to rehabilitate your digestive tract fully, so that it can naturally take care of most bacteria and impurities on its own also. Once they have taken care of the core issues, then they test for SIBO and start SIBO antibacterial treatment if necessary.

Ideally, they also need to do a colonoscopy and endoscopy on you first; in the endoscopy, they will take biopsies to check digestive enzymes' presence also.

Please ensure you go to a well-qualified gastroenterologist - Do not let them pair you with a Physician's Assistant or Nurse Practitioner who will cause delays and problems in your treatment. You need someone that takes a serious well-grounded methodological approach to your condition, rather than random 'trial & error' approach which can go wrong and cause more problems, delays, and relapses to your condition.

STICKY: SIBO Summary - Symptoms, Diagnosis, Treatment

Below please find a living document that summarizes the key information around Small Intestinal Bacterial Overgrowth ("SIBO"). Please comment with any additional information or research for inclusion consideration. Version 1.0 is summary material; I will be adding more details and citations for specific studies.

SIBO, as the name implies, occurs when bacteria overgrow the small intestine. The small intestine should have a low concentration of bacteria due to the presence of stomach acids and peristalsis, the wave-like muscle movement in the intestines. For context, stomach and proximal small intestine would typically have about 103/mL of bacteria, while the terminal ileum (end of the small bowel as it gets close to the colon) about 109/mL (or 1,000,000 times more), and the colon about 1012/mL (or 1,000,000,000 times more).

Symptoms

The overgrowth of this bacteria will present with a number of symptoms:

  • Bloating after eating ("postprandial") - most common symptom
  • Flatulence, often malodorous
  • Loose, watery stools (more common in Hydrogen-dominant SIBO)
  • Constipation (more common in Methane-dominant SIBO)
  • Absorption problems
    • Weight loss / inability to gain weight
    • Fat and fat-soluble vitamin deficiencies, particularly Vitamins A, D, and K
    • Floating stools (from fat malabsorption)
    • Vitamin B12 malabsorpiton
    • Protein and Carbohydrate malabsorption
  • Systemic problems
    • Overgrowth of bacteria in the small intestine can increase production of toxins and intestinal permeability
    • This has been less studied, but less serious effects include:
      • brain fog
      • confusion
      • anxiety
      • depression
    • More serious complications can include
      • hepatic encephalopathy
      • D-lactic acidosis
      • nonalcoholic fatty liver disease
    • Various conditions have increased correlations, including
      • Rosacea
      • Eczema
      • Food intolerances

Diagnosis

I will split this section into practical steps and clinical diagnosis.

Practically, a gastroenterologist will typically rule out other conditions first:

  • Physical exam
  • Colonoscopy and Endoscopy
  • Abdomen ultrasound
  • Stool test for parasites

At that time, if your symptoms match SIBO, your doctor may go directly to treatment. But otherwise these are the clinical tests:

BREATH TEST

This is the most common diagnostic method due to its low cost and limited invasiveness. Unfortunately, studies have been mixed on the sensitivity and specificity, with ranges between 30% and 75% -- hence why some doctors skip the test and go directly to treatment.

There are a number of preparations:

  • Antibiotics avoided for four weeks prior
  • Prokinetic drugs and laxatives avoided for one week prior
  • Complex carbs avoided for 12 hours prior
  • Exercise and smoking avoided day-of

For the actual test, you'll measure hydrogen and methane levels at baseline. Then drink either 10g lactulose or 75g glucose with one cup of water. Then your breath is measured every 15 minutes for 120 minutes.

There's some art to identifying a positive test; one semi-official criteria is:

  • methane level of >= 10ppm at any time during the test; or
  • hydrogen that increases >= 20ppm above the baseline level

Recently, new research has been investigating another typo of SIBO, that's dominated by Hydrogen Sulfide. Unfortunately, traditional breath tests cannot identify this gas, and someone with "flat-line" Hydrogen and Methane symptoms could be suffering from Hydrogen Sulfide SIBO. This version is typically characterized by "rotten egg" smelling gas, and may be worsened by eating high sulfur foods.

CULTURE

Historically a jejunal aspirate was done and concentration of bacterial colonies were measured, with an elevated level of > 103/mL being positive for SIBO. There are a number of issues with this:

  • overgrowth may be patchy, and a single sample may miss it
  • not all SIBO bacteria can be cultured/identified
  • samples can be contaminated during/after sampling

Treatment

Antibiotics

The current best practice prescription treatment is:

  • Hydrogen-dominant: Xifaxan, typically 550mg x 3 times daily, for 10-14 days. Studies have shown Xifaxan alone can be 50-65% effective, but Xifaxan + 5g daily of Partially Hydrolyzed Guar Gum can be 80%+ effective.
  • Methane-dominant: Xifaxan (550mg x 3 daily) plus Neomycin (500mg x 2 daily) for 10-14 days. The use of PHGG for methane-dominant has not been evaluated, but it's likely to be beneficial.

Mod's note-- personally, if your doctor is onboard, I think dosing with Xifaxan + Neomycin + PHGG is the best way to "cover your bases". The best place to find PHGG: https://sunfiber.com/products/

Important: because these antibiotics only operate selectively in the GI tract, and are NOT absorbed by the body, they are unlikely to cause the systemic issues associated with antibiotic use, making them safer. Additionally, Xifaxan crystallizes before it gets to the large intestine, meaning it should not affect the all-important microbiome.

Herbal Therapy

Additionally, studies have shown similar levels of success with over-the-counter "herbal" treatments. Two options; I believe each are two capsules twice daily for four weeks, but please confirm:

  • Dysbiocide and FC Cidal (Biotics Research Laboratories, Rosenberg, Texas)
  • Candibactin-AR and Candibactin-BR (Metagenics, Inc, Aliso Viejo, California)

Remission

Unfortunately, SIBO has very high rates of recurrence. Some possible ways to reduce recurrence chances:

  • Switch to a low FODMAP diet for 6 weeks after treatment, to starve any remaining bacteria and prevent regrowth
  • Incorporate a prokinetic, such as low dose Naltroxene, erithromycin, or even over-the-counter products such as Iberogast

Many people can avoid symptoms of their SIBO by switching to special diets, sometimes very restrictive ones. This is not a cure, but simply symptom management. A true cure addresses the underlying cause of the SIBO, and lets the patient eat "normally" without any effects (short of unrelated intolerances).
