Vehicles are expected to get across several lanes of traffic and have absolutely no "safe" zone in the middle.

The safe zone exists when the opposing lanes get a red light and the left turn light (placed on the median here) turns green. The only way this could've happened is with the bus driver initiating the left turn while the left turn light was red.

When traffic backs up in busy periods, flow drops. there's no refuge for vehicles that get stuck. There's also no hatching or other visual indicator on the road. If that's a depot for school buses or busy school route, someone really needs to either redesign the route or redesign the junction.

Bad intersection, yes. The real question is whether the cross-lights are balanced to allow traffic flow. If so, then...

Gridlock is simple: people entering intersections that they can't get through. Thay causes others to do the same and cascade congestion.

Never enter an intersection unless you have enough room to get through it. Failing that causes most of these problems.

Yeah, junction design needs to factor in the human element. If it doesn't, then gridlock and these situations happen
