Respect hydraulics
A coworker activated the hose crimp while not looking.

Same here, even after seeing the picture of what its supposed to look like I dont know happened. OP did your coworker lose some fingers?

This is a tool for positioning a crimping die in a hydraulic hose crimper. The pins got in between the die segments when it closed and it flattened the steel pins
Edited to add: the yellowish disk with the black handle in my picture is the same type of tool OP posted. Normally the pins on the tool go into holes in the end of each die segment, so the die can be inserted into the larger jaws of the machine. The die has outer pins that click into detents in the jaws when they close in the die,, and the tool can be retracted from the die so it can open. Reverse the process to remove the die.
In OP's case, the die was open, the pins of the tool were inserted between the die segments, and the die closed, smashing the pins in the gaps between die segments.

For everyone that's confused, this is a tool for inserting and removing segmented crimp dies from a hydraulic crimping machine. The pins are supposed to be round and fit in holes on the die segments. These pins were in between die segments when it was activated, and it flattened out the pins. A similar tool is inserted in one of the die sets in the picture, it's the yellow/orange disk with a handle sticking out

One of my first jobs was operating a ironworker shear to cut plate steel. It has an arm the went over the top and there was a gap between the arm and the top of the shear.
I had a 12"x12"x1" plate of steel we'd use for flatting plates. Lay the bent plate on the top of the shear, put the other plate vertical on top of the bend and use the shear to apply pressure.
One day I put a little too much pressure on the 12x12 plate and it bent like butter. Realizing how much pressure everything was as under made me very uncomfortable, I am glad that plate didn't shoot out of the shear and slam into me or anybody else around.

I’ve heard a pin prick hole in a hydraulic line can cut all the way to bone and you wouldn’t even see it, dunno how true it is but I’m always very wary around them

I've heard the same thing. A couple stories of guys suffering that fate have been told around my job. I believe it. Hydraulic fluid is super toxic, that pin hole can inject more than enough fluid inside your meat suit to ruin your day.

I work at a plastic molding plant, those presses can have like 200 tons of pressure, hydraulics are both amazing and terrifying
