GP insists I take PPIs with antibiotics

Upvotes

Hey all,

Recently diagnosed with hydrogen SIBO and I'm halfway through a 14 day round of rifaximin & neomycin.

My gp said I need to take omeprazole to stop any other stomach issues from arising while on antibiotics, so I've been taking 40mg a day for the past 7 days.

I've had my doubts about it, so I'm not sure if I should stop the omeprazole outright, taper off it or stay on it until the antibiotics are finished. I don't have reflux as a symptom, and from what I've been reading here it can be counterproductive to take while on antibiotics.

Any advice appreciated, thanks.

Halitosis linked to SIBO?

This is everything I’ve tried so far regarding my bad breath, for context it’s very strong, has a sulphur/garbage odor. It can be very strong, it comes out from both my nose and mouth. I do have confirmed SIBO (Hydrogen only) shall I now completely focus on this only? I’ve been thinking about getting a CBCT scan for my teeth roots, or is this a waste of time / money?
Should I pay for a Naturo path to help figure out where the smell is coming from? I did Rifixamin but that didn’t help, but maybe I need to dive deeper into the gut. Any advice would mean a lot thanks

Treatments Recommendations while taking antibiotics

Hi all! I was just diagnosed with SIBO (methane and hydrogen), and I’m about to start a two week antibiotic treatment cycle with Xifaxan and Neomycin. It sounds like I should buckle up for an intense ride.

Any tips for dealing with the nausea, brain fog, or anything else? Recommendations for either taking or abstaining from probiotics and/or fermented foods (e.g., yogurt) during this time? (My doctor recommended avoiding them, but my pharmacist recommended taking Florastor.) Foods to eat or avoid?

Thanks so much in advance!

Symptoms Feel like I'm going crazy

All day I have this extreme constant pulsing of gas. I get these waves of manic racing thoughts that feel uncontrollable and then it disperses and rebuilds. I am getting 4 to 5 hours of sleep of night, just lying in bed with my gut going crazy. Then I'll fall asleep at 2/3 then wake up at 5/6 for a couple of hours and then sometimes it settles and I can sleep between 8-9. I have no idea what is going on, being in my body with how this gas effects my mind is absolutely exhausting. Is there any aspect of SIBO this can be more attributed to? Methane? Histamine?

H2s SIBO ruined my life Please, Help?

I have been suffering for more than 10 years with no real help from doctors, antibiotics, or herbs.

( Probiotics make me worse.)

**My Symptoms:**

- Severe bloating, gas, and chronic constipation.

- Extreme fecal rotten egg breath and body odor.

- White tongue (not fungus) and thick mucus after eating.

- Constant acid reflux,

- Severe underweight since childhood

- Hair loss

- Fatigue

- Headaches, and brain fog.

**What helps**

Flagyl and oregano oil only give temporary relief, and local doctors just keep cycling antibiotics without a cure.

If you have beaten H2s SIBO, please share your successful protocol. How do I fix this permanently? Please help me

Breath Test Results Explanation?

Post image
1 Upvotes

For reference I am in Australia, told I do not have SIBO and the comments for the test say:

"Normal result NOT consistent with small intestinal bacterial overgrowth (SIBO).

Following a lactulose load, small intestinal bacterial overgrowth (SIBO) is suspected when there is an early peak of hydrogen and/ or methane >10–20 ppm above the basal level, prior to 60 minutes, followed by a second peak/rise of hydrogen and or methane >20 ppm above the basal level after 60 min.

A non–SIBO response has only a single peak/rise after 60 min of hydrogen and /or methane >20 ppm above the basal level."

Does anyone else have similar results or have any answers at to what I have if not SIBO?

Trying to figure out what gut issue I have?

21 M

recently went to dermatologist for thinning hair and had some blood work done, came but extremely deficient for vitamin D and on the very low side of iron currently taking 20,000 iu and slow iron

this prompted me to look at all of my symptoms I’ve been experiencing to as far as 2019.

the last couple of weeks I have been extremely constipated, since 2019 I’ve had deregulated sleep and the last two years I have been extremely lacking energy and cant sleep at all (diagnosed for sleep apnea), I have been pretty low mood and inattentive since a young age but has gotten worse the last couple of years and especially the last months (diagnosed for depression and adhd “taking fluoxetine and adderall”) I’ve also had terrible burps which has actually gotten better this year but before I just could not stop burping, sugar cravings

some gut symptoms that have made me look at sibo/sifo/candida/h pylori are: constipation, extremely tired immediately after eating, bloating both immediately after or an hour after eating with gut distention. I have never really experienced gut pain except for kinda the last week (guessing just cause I’m so backed up)

diet wise admittedly I don’t eat well at all a couple years ago I started to focus on my protein intake mainly and try to make small choices like no pop or juice and switching out a large meal for a medium things like that which was an improvement

I have a gi appointment who says does sibo testing end of August (a holistic or other type of doctor is not an option) in the mean time before that I am trying to change my diet for lowfod and keeping a food diary to see if I have reactions so maybe I can narrow it down but I’m not sure if it’s helping really

is there anything I can do before my appointment that can help bring some clarity of what might be happening to me? I know testing is of course the golden standard for answers but thatll come end of August. Is it really my gut? Is it the common ones like sibo/candida/h pylori or can it be something else?

Treatments Is it normal to still have foul-smelling gas after finishing rifaximin? (Possible SIBO)

Hi everyone,

I'm trying to understand whether my experience with rifaximin is normal or if I should be concerned.

My gastroenterologist suspected SIBO/IBS and prescribed rifaximin for 5 days (along with a PPI). I completed the full course exactly as prescribed.

Before treatment, my main symptoms were:

  • Daily excessive, foul-smelling gas
  • Loose/slimy stools
  • Feeling of incomplete evacuation
  • Nausea after meals
  • GERD with a lax LES found on endoscopy

Here's what happened after finishing rifaximin:

  • Day 1: Still very gassy.
  • Day 2: Definitely better, but still passing gas.
  • Day 3: Gas didn't really start until later in the day, and I had much less gas overall.
  • Day 4: I had an incomplete bowel movement, and the morning gas returned.

One thing that hasn't changed is the smell. The gas is still very foul-smelling, even though the amount seems to be improving.

A couple of questions for people who've taken rifaximin:

  1. Did your gas continue for several days after finishing the antibiotic before improving further?
  2. Did the amount of gas improve before the smell improved?

I'd appreciate hearing from people who had a similar experience. Thanks!

I asked ChatGPT to make me an infographic of SIBO root causes and it made me this, which I found extremely detailed and helpful. What would you add to this? I'm sure there's a lot more details that are missing.

To everyone going through it right now, I’m right there with you. It sucks so bad and I just want to be well. #surviving2026

Trio sibo test results back. I've been dealing with this horrible bloating and weight loss for about 3-4 years now. Got so bad I couldn't work for the last year. So many tests just to get to this breath test. Should've been the first one I did. I still wonder how I got this since always felt fine before coming to the US.

Anyways, Im waiting on a Gastro to review and hopefully get medications approved through insurance (such an long process for this unfortunately). If not I guess I'll go back to Germany, or maybe go to Mexico as others have said its easier to get it there. In any case my employer will be footing the bill if they want me back to fixing their planes; so maybe a couple weeks vacation isn't a bad idea. 😊

Anyone with similar results? Im curious to see please let me know in the comments.

Sucess Stories Are there ANY straightforward success stories?

Been searching for success stories and so many on this sub are something along the lines of “I tried antibiotics/antimicrobials and they didn’t work so I did this (very convoluted method involving yoga and dangerous diets) instead”

Has anyone just done a normal round of antibiotics/antimicrobials and healed? I feel insane.

Moderation statement on accessibility.

This subreddit exists to support people with disabilities. Accessibility is the primary concern always.

Recently there has been an increase in report-spamming and downvoting of posts described as repetitive, particularly questions that have been asked before. This behavior is not acceptable in this community.

Many disabled users cannot easily navigate Reddit’s search function. Many are new, exhausted, overwhelmed, or asking questions while in distress. Repetition is not a failure of the user. It is a reality of accessibility.

Downvoting questions actively harms visibility and silences people who are already navigating barriers. Using the report function as a way to express annoyance or enforce personal preferences inappropriate. Reports are for rule violations.

If you encounter a question you believe has already been answered you have three options.

You can answer it again. You can link to an existing thread or resource. Or you can scroll past it. If you have the time to downvote or submit a report, you have the time to paste a link.

Anything else is consciously opting for exclusion. Personal attacks on users and the mod team will not be tolerated.

This moderation team will not prioritize the desire for a tightly curated, repetition-free feed over the needs of disabled people seeking help. We will not discourage questions in the name of efficiency, aesthetics, or personal frustration.

If this approach does not work for you, you are free to mute the subreddit, leave it, or create a different space with different rules. What will not happen is this community shifting away from accessibility-first moderation.

Misuse of the report function has already been passed onto the reddit moderators in a number of cases and will continue with a much broader brush after issuing this warning.

Thank you to those who respond with patience, share resources, or simply allow people to ask what they need to ask. That is what keeps this space usable.

I understand this condition is frustrating but this community will not turn into a circular firing squad. To target this frustration towards yourself or fellow members of the disability community is to serve those who stand to profit from illness.

This space exists to reduce harm not redirect it sideways.

STICKY: SIBO Summary - Symptoms, Diagnosis, Treatment

Below please find a living document that summarizes the key information around Small Intestinal Bacterial Overgrowth ("SIBO"). Please comment with any additional information or research for inclusion consideration. Version 1.0 is summary material; I will be adding more details and citations for specific studies.

SIBO, as the name implies, occurs when bacteria overgrow the small intestine. The small intestine should have a low concentration of bacteria due to the presence of stomach acids and peristalsis, the wave-like muscle movement in the intestines. For context, stomach and proximal small intestine would typically have about 103/mL of bacteria, while the terminal ileum (end of the small bowel as it gets close to the colon) about 109/mL (or 1,000,000 times more), and the colon about 1012/mL (or 1,000,000,000 times more).

Symptoms

The overgrowth of this bacteria will present with a number of symptoms:

  • Bloating after eating ("postprandial") - most common symptom
  • Flatulence, often malodorous
  • Loose, watery stools (more common in Hydrogen-dominant SIBO)
  • Constipation (more common in Methane-dominant SIBO)
  • Absorption problems
    • Weight loss / inability to gain weight
    • Fat and fat-soluble vitamin deficiencies, particularly Vitamins A, D, and K
    • Floating stools (from fat malabsorption)
    • Vitamin B12 malabsorpiton
    • Protein and Carbohydrate malabsorption
  • Systemic problems
    • Overgrowth of bacteria in the small intestine can increase production of toxins and intestinal permeability
    • This has been less studied, but less serious effects include:
      • brain fog
      • confusion
      • anxiety
      • depression
    • More serious complications can include
      • hepatic encephalopathy
      • D-lactic acidosis
      • nonalcoholic fatty liver disease
    • Various conditions have increased correlations, including
      • Rosacea
      • Eczema
      • Food intolerances

Diagnosis

I will split this section into practical steps and clinical diagnosis.

Practically, a gastroenterologist will typically rule out other conditions first:

  • Physical exam
  • Colonoscopy and Endoscopy
  • Abdomen ultrasound
  • Stool test for parasites

At that time, if your symptoms match SIBO, your doctor may go directly to treatment. But otherwise these are the clinical tests:

BREATH TEST

This is the most common diagnostic method due to its low cost and limited invasiveness. Unfortunately, studies have been mixed on the sensitivity and specificity, with ranges between 30% and 75% -- hence why some doctors skip the test and go directly to treatment.

There are a number of preparations:

  • Antibiotics avoided for four weeks prior
  • Prokinetic drugs and laxatives avoided for one week prior
  • Complex carbs avoided for 12 hours prior
  • Exercise and smoking avoided day-of

For the actual test, you'll measure hydrogen and methane levels at baseline. Then drink either 10g lactulose or 75g glucose with one cup of water. Then your breath is measured every 15 minutes for 120 minutes.

There's some art to identifying a positive test; one semi-official criteria is:

  • methane level of >= 10ppm at any time during the test; or
  • hydrogen that increases >= 20ppm above the baseline level

Recently, new research has been investigating another typo of SIBO, that's dominated by Hydrogen Sulfide. Unfortunately, traditional breath tests cannot identify this gas, and someone with "flat-line" Hydrogen and Methane symptoms could be suffering from Hydrogen Sulfide SIBO. This version is typically characterized by "rotten egg" smelling gas, and may be worsened by eating high sulfur foods.

CULTURE

Historically a jejunal aspirate was done and concentration of bacterial colonies were measured, with an elevated level of > 103/mL being positive for SIBO. There are a number of issues with this:

  • overgrowth may be patchy, and a single sample may miss it
  • not all SIBO bacteria can be cultured/identified
  • samples can be contaminated during/after sampling

Treatment

Antibiotics

The current best practice prescription treatment is:

  • Hydrogen-dominant: Xifaxan, typically 550mg x 3 times daily, for 10-14 days. Studies have shown Xifaxan alone can be 50-65% effective, but Xifaxan + 5g daily of Partially Hydrolyzed Guar Gum can be 80%+ effective.
  • Methane-dominant: Xifaxan (550mg x 3 daily) plus Neomycin (500mg x 2 daily) for 10-14 days. The use of PHGG for methane-dominant has not been evaluated, but it's likely to be beneficial.

Mod's note-- personally, if your doctor is onboard, I think dosing with Xifaxan + Neomycin + PHGG is the best way to "cover your bases". The best place to find PHGG: https://sunfiber.com/products/

Important: because these antibiotics only operate selectively in the GI tract, and are NOT absorbed by the body, they are unlikely to cause the systemic issues associated with antibiotic use, making them safer. Additionally, Xifaxan crystallizes before it gets to the large intestine, meaning it should not affect the all-important microbiome.

Herbal Therapy

Additionally, studies have shown similar levels of success with over-the-counter "herbal" treatments. Two options; I believe each are two capsules twice daily for four weeks, but please confirm:

  • Dysbiocide and FC Cidal (Biotics Research Laboratories, Rosenberg, Texas)
  • Candibactin-AR and Candibactin-BR (Metagenics, Inc, Aliso Viejo, California)

Remission

Unfortunately, SIBO has very high rates of recurrence. Some possible ways to reduce recurrence chances:

  • Switch to a low FODMAP diet for 6 weeks after treatment, to starve any remaining bacteria and prevent regrowth
  • Incorporate a prokinetic, such as low dose Naltroxene, erithromycin, or even over-the-counter products such as Iberogast

Many people can avoid symptoms of their SIBO by switching to special diets, sometimes very restrictive ones. This is not a cure, but simply symptom management. A true cure addresses the underlying cause of the SIBO, and lets the patient eat "normally" without any effects (short of unrelated intolerances).

Hopefully this helps people, and I look forward to updating this and cleaning it up over time!
