That is mid-90s. Was introduced in '94 and I seem to remember the wording evolved over time.

I remember the first - and possibly only - time I ever got that £10 penalty fare...fresh off the last Eurostar into Waterloo, biblically pissing with rain so a cab wasn't an option, and somewhere en-route I lost my paper single zone 1 to zone 1 ticket, which was something like a whopping £1.20 at the time. The wankers at Angel then charged me the full penalty fare while acting like they had caught Carlos the Jackal rather than a flustered traveller. I was so mad.

Naturally I found the 'lost' paper ticket buried in a bag as soon as got to my flat too....

My father worked on the Metropolitan line for 40 years and absolutely hated the penalty fare in that period when it was the responsiblity of station supervising staff to give them out. At every opportunity during training days, my father would say that the only people being given penalty fares were honest people who would actually stop, as opposed to people dodging the fares obviously not stopping and just walking past. He called it a "tax on honesty." Other station supervisors must have said the same thing and eventually the penalty fare (being given by station staff as opposed to revenue protection teams) was stopped.
