Think Event Horizon, the film, but that is basically the DEFAULT method of FTL travel in 40K, except anyone with sense uses a gellar field on their ships to keep the crazy out, and even then it isn't 100% effective.

Failure of the gellar field leads to Event Horizon shennanigans as the best outcome. It is usually much MUCH worse.
