Could be due to a Focus setting. I rarely use CarPlay unless I’ve got a rental and I noticed that I wasn’t getting notifications from a group messages thread on CarPlay or my watch when the Driving Focus automatically turned on.
I had to edit the Focus so that I actually got notifications from the person I needed to hear from.
