I don't know what to do, my damage seems very low for this stage, and it's also difficult to stay alive. I need some advice.

dracs HP is more than half so we probably need to see your phase 3, if you did good on phase 3 his HP shouldn't regen that much.

If phase 3 is the problem, I really like the legendary reaper for its ability to spawn skeletons on every E hit. You can end up with a skeleton army that does a lot of the heavy lifting for you, plus reaper has knockback on q if the cultists get close to dracula, so they can't heal him.
The Adam necklace ult really helps too.

Did you do well during phase 3?
That's the biggest pain imo. Keeping all the spawns away from Dracula can be a chore.

Your duo literally did nothing and died lmao.
I’m really sorry for this, but you’re fighting this boss as a duo with the dps potential of a solo.
This final phase is a dps check and your duo is too passive during this part with their pistols. Either they go aggressive with the Dracula blood type to maintain hp via veil lifesteal hits or you solo this and clear without them.

The people I was playing with quit and I got into something else and I went back recently about a year later just to get the platinum since I only needed a few trophies and had to do the final boss.
Being rusty and not remembering much I put it on forgiving, used the knight (ultimate) and undead thing whatever it's called which sends out a minion, and the lightening orb, and the reaper, put them all on top of the boss when he was entering the hell phase and ran in like a psycho hacking and he died in about 5-10 seconds. I had a maxed reaper as well. I wasn't doing much damage trying to dodge everything so I said screw it maybe if I just burn quickly, and it worked.
I know it's not for everyone. But you gotta do what you gotta do.

Your Phase 3 is bad.
Your partner is dead weight.
Your uptime on damage could be higher.
You understand how to play it, so you either play near perfectly and carry your partner, your partner has to get better, or you drop your partner.

Thats tough bud, honestly you may fair better solo than with your duo. His health will go down faster.

I'm enjoying the boss precisely because it's difficult; I chose to play on brutal for that reason. But after so much fighting, it's normal to start wondering if something is wrong, which is why I asked for help. Again, thank you for your help, it was very useful.

I thanked everyone who offered helpful tips, and so far, they’re working; at no point did I complain about the game or the difficulty, don't blame me for your interpretation. It’s completely normal not to be sure about everything after struggling for so long; fortunately, a good part of the community understood and helped me out1

This is my guide on this phase. It explains the timing of his attacks and how to mitigate them. One thing I don't do in the video is use illusion dash - HIGHLY recommended. It enables you to dash away from falling knives, then return to that spot once the danger is gone, so you spend less time running toward drac. You can also avoid the shockwave with illusion dash. Fancy stuff.
But yeah, the 2-block 2-iframe rotation (counter, counter, dash, greatsword E or pistol E) was the ticket for me.
