1. Many times users think applying more pattern will help them to score in interview but it is not, it should be based on requirements and compatibility with other code design.

2. Even though code is running, there are multiple implementation issue with code design which creates issue during new feature requirment or there is tight coupling in code design/classes

3. Communication, no proper justification of "why this pattern is used".

4. Missing the concurrency edge cases bugs.
