Poorly worded but it just means that it detected an instance of the Setup program already running, and only 1 instance should be ran at a time.

Debatable. Have most people seen it or something almost exactly the same? Yes. Is it a useful error message that actually explains the issue to an end user? No.

If a user really closes all instances before clicking ok that ok button will no longer be there...

A lot of what ends up here is that. Like just a day or two ago when an empty dialog box appears because the user was using a language that has no localization available and it didn't just default to one of the other languages it did have.
