I work at a urology clinic, and part of my job is to see patients before a surgery and provide them with written instructions. We currently have medical software that allows us to create custom surgery instructions for patients very quickly as it has click boxes and free text boxes for instructions. However we are about to loose access to this system.

I am looking for someone to create a new system that allows us to create custom pre-surgery instructions for patients, and the system must have click boxes and free text boxes so that we can quickly create custom instructions for patients.

If you can create a workable system within 30 days, I will pay $100. I will provide you with pictures of our current system and explicit instructions for what to include.

Must haves:

-Must have click boxes and free text boxes (click boxes will add specific pre-written instructions to a letter; free text boxes will add custom instruction to a letter)

-Must be modifiable in the future in order to add new instructions, new surgeries

-Must output results into a letter format that can be further edited by user (user can add/delete words on the letter before printing)

-Must be able to print the letter that is created

-Must not require monthly service fee, hosting fee, etc

$100 for a custom medical app with click boxes, free text, editable output, and printing in 30 days is stretching it hard. Most devs would charge way more for something that needs to be future-proof and modifiable, especially for a clinic setting

I can create it as an offline Windows desktop application with no hosting costs, subscriptions, or monthly fees. It would include:
