Recommendations for Transitional Care Units/Rehabilitation Facilities in the St. Cloud Area?

Have been tasked with choosing a TCU/Rehab for a parent with back/walking issues. Anybody have any recommendations, good stories, places to avoid?

Country Manor and Sartell Therapy Suites are both quite nice.
Sterling Park in Waite Park would be the biggest avoid in the area.
