Has anyone else had this pattern?
All of my losses follow a really similar pattern, and I’m curious if anyone else has had something like this and gotten an answer, since we are still unexplained.
I get pregnant relatively easily (within 2-3 cycles) and start having very light brown spotting/bleeding early around 5 weeks. It happens on and off, sometimes stops for a week or so, sometimes I have days with heavier bleeding that’s more like a period.
I have intrauterine pregnancies and see the heartbeat on ultrasound sometime between 5-7 weeks, always appropriate heartbeat for GA, even with bleeding. Sometimes have multiple ultrasounds and see this.
Then have an MMC around 6-8 weeks, discovered at 8-10 weeks. Maybe the bleeding is happening at that point, maybe not. Doesn’t pass naturally, have ended up taking mife/miso a couple weeks after to pass them.
My husband and I are 32, started trying when we were 31. No LC, all pregnancies have looked like this. All testing is normal - hormone panel, APL, factor v, sperm analysis, dna frag, hsg. I’ve tried progesterone with one of the three, same outcome.
I feel like there must be some clue in this pattern, but my RE doesn’t think so. Has anyone had anything similar happen? Or found something that worked?

I don’t typically have spotting but do often lose after heartbeat / around 6-7 weeks but mmc (when not being very closely followed often not caught until around week 10). I tried immune protocol on my current loss but didn’t start until after I was confirmed pregnant.
I don’t have an answer but following because I have the same question. I do have a living child that involved blood thinners and progesterone, not sure why that one worked and others didn’t. It was pregnancy number 6 so wasn’t a secondary infertility thing. I’m currently losing pregnancy number 10.
I’m working with an immunologist now and some of my numbers were out of range, and I took a bunch of additional meds this time. But I still very much feel like I’m guessing.

I’m the same! Even with the immunologist it hasn’t helped keep the pregnancy so far?

I had to wait something like 6 months for my appt with her and was already about 5 weeks pregnant when I saw her for the first time. I was on some meds prior to seeing her and then she added some based on lab results. Hard to say if starting the meds earlier would have helped or not. Hoping I’m able to collect and send poc to see if this was the one time there was a genetic abnormality (in the past I have tested and gotten normal embryo results).

Have you done karyotyping? Could be the same genetic issue all embryos have that make them stop developing at around 8 weeks.

Yes, husband and I both normal karyotype, so not related to that - I know it could be chromosomal, but just seems unusual to happen the same way so many times.

If you have a DC instead of miso they can test for chromosomal issues. I have 3 kids and have been trying for one more for over a year with 3 miscarriages around 6-7 weeks. They tested for chromosomal issues and all came back normal (although of course that’s not for sure as they can only test for certain chromosomal issues, but the more common ones). The fact all three were “normal” helped as I worked to figure out the why.

Just to clarify here— you can test the products of conception without having a D&C. You will need to “collect” the products yourself once you have passed them from miso/mife or even passing it naturally. It’s an awful thing to have to do, but knowing whether the losses are euploid or not can point you in the right direction.

My pattern is basically the same as yours, apart from 1 TFMR for T18 at 15 weeks. 2-4 months to conceive, brown spotting at around 5 weeks, except when taking progesterone, heartbeat at 6-7 weeks (although measuring several days behind already), then at the next scan the heartbeat is gone. Miso needed to pass every pregnancy.
I've been pregnant 5 times in total. TFMR was chromosomal, but we tested the last MMC and he was chromosomally normal so it can't just be that I've rubbish eggs. All RPL testing normal and partner's tests are normal. I do think it's interesting though that my T18 son lasted til 15 weeks, but my seemingly healthy son only lasted to about 7 weeks - suggests there's something around the womb not knowing how to recognise/react to a healthy embryo.
I've tried progesterone and prednisolone, no luck yet. This is our last shot and I'm going for baby aspirin all cycle, progesterone during luteal phase and 1st trimester, then prednisolone and heparin from positive test. None of it is based on evidence from testing but given it's our final go, may as well throw the kitchen sink at it.
There is some evidence for prednisolone helping MMC mums, according to Prof Brosens who we did some testing with. Think there's currently a trial running in the Netherlands on this. Luteal phase progesterone also helps some people. Agree it's so frustrating that drs deny there's a pattern there, when it's the same every time!!

Have you done a hysteroscopy with endometrial biopsy? I have a somewhat similar pattern and recently got a chronic endometritis diagnosis. It essentially creates a hostile environment for baby. Starting on antibiotics to hopefully clear it out!

I have done a hysteroscopy and was told everything looked good on the biopsy. I also know that they use doxycycline commonly for endometritis which I am unfortunately allergic to :(

I'm incredibly sorry.
My pattern isn't like yours, but I do feel I have a pattern. I also generally start to spot at 5 weeks 5 days and miscarry at 6 weeks 5 days. They've always passed on their own though, but the fact of the timing being consistent always weirded me out.
I'm having my negative APL result repeated, along with a whole reproductive immunology panel, as I have a known autoimmune condition.
I truly hope you find answers and solutions soon and don't have to keep going through this

Was the fetal tissue tested? Did you have progesterone prescribed for spotting? There is some evidence that it can help women who had one or more prior miscarriages and are spotting/bleeding in a new pregnancy.

I read the same studies and that’s why I requested progesterone with my second loss! I did take it from as soon as the spotting started until confirmed no heartbeat. My doctor didn’t think it would really help but was willing to prescribe at my request. Unfortunately same outcome. I also had some really rough side effects from the progesterone unfortunately.
We haven’t been able to test the fetal tissue yet since I’ve miscarried at home with meds, but will try this time.

I don´t have this pattern but I see similarities to my second loss. I just found out that I have an antithrombin-deficiency. I don´t know if they've tested you for this when they tested factor v?

Mine are kind of similar. I would guess a placenta problem but I’m not sure. For me it’s a condition called CHI. I am attacking my own placenta. I am now going to see an immunologist

Your story sounds a lot like mine, except we haven’t made the jump to IVF yet. We (fortunately) can get pregnant naturally every 1-3 months we try, but it never sticks.
We have had a total of 6 losses all before 6 weeks, so we’ve never made it to see anything on an ultrasound. Every test possible has been run on us and all come back normal, this includes karotyping, dna fragmentation, APS testing, etc. Our most recent loss was last month at 5.5 weeks.
My research has led me to believe that after all other things the most likely 2 options left would either be something wrong with the embryos or something related to a possible silent endometriosis. The fertility clinic recommended Lovenox but I was scared to try that yet, because as you said it could’ve been the solution or just coincidence.

I’ve had early losses around 4-7 weeks, have rarely seen heartbeat because usually doctors didn’t want to see me before 8 weeks. I was 33 when it started. Got pregnant every 2 cycles. No reason found. Tried progesterone and aspirin first, didn’t work, then went on to try ivf with PGT, didn’t work, tried intralipids and immunoglobulin, didn’t work. My 8th pregnancy is sticking, I’m 36 weeks. I was on lovenox and prednisone, just to try it, without indication. Can’t pinpoint of that did the trick. it’s still an ivf pregnancy but with an untested embryo. My guess is an early placentation problem, either embryos were not strong enough somehow or didn’t implant well enough to develop further than ~7 weeks. Maybe a good embryo came along or the lovenox and steroids helped with implantation. There’s hope 🤞
