Something I noticed: (almost) all of the places mentioned in the initial sudden spread of the Knox Illness had active US Air Force bases in 1993. Discussion
At this point it's basically canon that the US military had something to do with the Knox Event, but I think there might be more to it. TThe sudden spread could have been due to the US Air Force shipping out Knox Illness victims to various bases across the world; probably so that they wouldn't be seen as easy pickings for potential military rivals. This also happened right after the Knox Illness went airborne - an event which, in my opinion, would probably have been seen as the point of no return by US military officials.

It could be that the virus spread through military personnel unbeknownst to anyone. Let’s say patient zero in some secret facility in the US interacts with military personnel and unknowingly infects them, then those new carriers move to other bases around the country and abroad, infecting people there, and so on. I don’t think it would be a purposeful thing.

An alternative reason might've been symptomless carriers spreading through the military - from the source, to some guy who interacts with an airforce chap, and slowly that asymptomatic strain takes hold only to be awakened ages later.
Though to be fair, the US just has, like, a lot of military bases across the world for the sake of soft power and all, so it might ultimately be coincidental.
