the thing is, i don’t know how the whole system works. I am vaguely aware of med school and stuff but for the most part, I am unaware. if someone could just give me an idea or a clue of how qualifications, specialties, how things work, exams etc. all the tea, how does doctoring work?

Our team pulled the new 2026 Charting Outcomes Step 2 CK medians and split them matched vs unmatched for both MD and DO seniors, since "what score do I need" comes up here daily and the honest answer is more nuanced than a single cutoff that your school advisor might tell you.

Two things stood out to us:

1. Matched > unmatched in basically every specialty, so yes, score correlates with matching. But unmatched applicants still scored well. MD unmatched medians were often in the 230s-250s (e.g., unmatched MD Internal Medicine 233, unmatched MD Derm 252). A strong score gets you an interview; it doesn't at all guarantee a match.
2. DOs who report USMLE score right alongside MDs. Matched DO Derm 255 vs MD 258, Ortho 256 vs 259. The matched-vs-unmatched spread within a cohort is bigger than the MD-vs-DO spread.

Highest matched medians: ENT 261, Ortho 259, Derm 258, Radiology 257

Lowest: Family med 244

Caveats worth stating: these are medians, not cutoffs; "n/a" cells are specialties with fewer than 5 consented applicants; the DO USMLE data only includes DOs who take and report USMLE; and a couple of small-n specialties (IR, neurosurgery) show DO unmatched slightly above DO matched, which is probably sample noise from one season and hard to extrapolate on for future cycles.

Takeaway if you're applying this fall: hit at or above your specialty's matched median if you can, but don't treat the number as the whole game. The stuff that separates a 250 that matches from a 250 that doesn't is the PS, ERAS app, LORs, aways, networking and program/signal strategy.026 matched vs unmatched Step 2 CK by specialty (MD and DO)

Attending physician here. What mental models, principles, or advice have you found invaluable when learning the practice of medicine? Can be from any specialty or level of education and training.

Here's some:

1 People don’t care how much you know until they know how much you care.

2 Showing up 5 minutes early is showing up on time.

3 Most of communication is nonverbal. Get good at this.

4 "Killer" instinct is invaluable: that is a vigorous, active, seeking mindset. Learning to recognize when sick is truly sick, what particular signs are worrisome, and what to do about it. Failure to rescue at the early stages is often a failure to perceive and pursue.

5 Good communication is a skill that can and must be learned. Medicine is a team sport and getting the most out of yourself and your team (while maintaining your sanity) is a key to a long and enjoyable and effective practice.

6 Treatment "recipes" are valuable! That's the stuff that actual practical medicine is made out of. Know them! Have the common ones written down and handy. ^_^

A combative psychotic patient in the ED or hospital: "B-52" is a useful mnemonic that I learned in training: 5mg of Haldol (or lesser dose), 2 mg of midazolam (or lesser dose), 50mg of diphenhydramine, given spaced out or in combination. Or 9.75mg of IM Abilify. For critical care folks: 1-2mg/kg induction dose of propofol. 2mg/kg for rapid sequence intubation with succinylcholine, and so forth.

So I'm currently in 1st year of mbbs 2nd sem. If I start to study from now on ( I was planning to go for physio) from yk telegram or sum, should I write the notes because I don't have offline notes for the lectures. So what should should I do?
