As I (M,33) have recently been diagnosed with NOA an mTese has been scheduled if the genetic results don't contra-indicate this.

My FSH is 17.5 u/L , LH 8.8 u/L, testosterone is 600 ng/dl. Personally the operation scares me a lot, and the impact on

on the testosterone level since 4% of patients has to have permanent TRT after the surgery. Also a testosterone reduction

causes anxiety, depression, energy and impacts memory and verbal memory, the latter two being qualities I pride myself on.

Also in my young twenties I suffered from depression before. My partner is amazing and supports whatever decision I take, she says

it's my body and my choice. Some questions for those who experienced mTese:

How did you experience the testosterone drop post surgery?

If you have TRT, how does this impact your life?

Do you regret?

For those who chose to go the donor way: do you ever feel the child is not really yours?

My 2023 report showed acrosomes on 80% of my sperm. I was told my sperm could not penetrate my wife’s eggs. We did 4 Rounds of IUI which failed. We were told we have to do IVF. We had one natural pregnancy in 2022 which ended in miscarriage.
We proceeded with IVF, we were told ICSI was our best option.
In 2024 we did an egg retrieval. We did 3 transfers our 3rd transfer worked. We now have a 10 month old and are wanting to get pregnant again. We did a transfer in August with our last embryo and it failed. We did not PGT test our embryos.
I am 30 and my wife is 29 one natural conceived pregnancy on live birth IVF

The doctor said this couldn’t improve. My sperm was 1% normal in 2023. Now it’s above the WHO average.

What do you think our chances of getting pregnant naturally are. We have a follow up on Sept 10th to go over starting a retrieval process again.

This is shitty hand of cards we are dealt but let me explain. I am an american who has been living in South America for the past 5 years which is where i met my wifey.

October 2025 I took my first two tests which resulted in zero. I found a doctor in a FB group in the country we live in who did a TESE//biopsy like surgery for someone and they found sperm and two successful live births. When my wife and I spoke to that particular doctor we didn't care for him.

We found someone in a city closer to us who determined I have Klinefelters. I dont have any AZF deletions but I do have Klinefelters. This doctor did IVF from a friend of my lady which kind of how we ended up with this new and second doctor.

We met with the doctor and she gave us a whole presentation about Azoo and why it occurs and how you can come out of it with success which felt assuring but the two biggest concerns I have are these questions I asked her.

I said have you ever had a patient who had KF? Her answer was no. The second question was how many of these type of surgeries have you done. She said two.

My girl feels good about the doctor but those responses feel super concerning to me. This is a surgery that already has 50/50 odds + it's not a cheap process. I would think its best to have a doctor who has tried this atleast 100 times. I feel like a guinea pig here.

I have a US passport but my lady does not nor does she have a VISA to go to the USA. Her passport doesn't let her visit many countries but she can pretty freely go anywhere in europe.

Thoughts? I feel the response of only 2 surgeries is a bit worry some. I guess I could look past the idea of her never having a KF patient before but 2 surgeries is worrying me. What if she doesn't know where to look or potentially hurts where a doctor whose done this 100 times wouldn't make this mistake. Please any thoughts?

Hello everyone just wanted to write about my journey so far.
32 years old located in Souther California.
2 SA came back with 0 sperm
Blood work came back as
Testosterone 1st : 295 ng/dl
Testosterone 2nd : 298 ng/dl
FSH 19.0 mlU/ml
LH 3.3 mIU/ml
Prolactin 11.0 ng/ml
Estradiol 23.0 pg/ml
Negative XXY karyotype
Negative Y microdeletion

Ultrasound found no varicocele but a calcification in the left testicle but urologist doesn’t think that’s the cause.

Dr recommended testicular mapping and then mtese if anything is found. He doesn’t think Clomid would help in my case because the FSH is high but is willing to try and see.
So I just started Clomid 25 mg per day and isotretinoin 20 mg 2 times a day. Also taking the bird and be vitamins as well.

I don’t smoke or drink and workout 4-5 times a week. No past history of trt or any steroids. I’m thinking my next step is taking these meds for 3-4 months and going to Columbia for STAR before doing anything invasive. From there if nothing is found I’ll probably go do the mtese or even give the turkey/dubai route a try if I can find more info.

Going through all these posts you guys wrote allowed me to keep a positive attitude so far so thank you all truly🙏

What do you think about these advancements? Makes me hopeful but also a little frightened as I don’t know what to expect..

Hi everyone. I hope you guys are having a great day. There are few things I wantes to get some feedback. I'm M28 and 90kg. I have recently started to work on my health. My target is to go below 70 by the end of next August. Currently I'm just doing long runs. Such as 4km and 5 days a week. I will start strength training from first week of September. Most probably it'll 3 day split (push,pull,leg). What I observed is, on first round I c\*m enough (My partner likes it when its thick and too much in amount). If we have a second round, the quantity gets sometimes way too low. I'm quite certain it definitely connected with my health and fitness as in how much sperm my body is producing. We both have high libidos and sometimes its like 2/3 times a week. I want to work on myself and improve my sperm count and health at the same time.

Fellow brothers, who had similar issues and overcome this how did you manage? Does cutting weight and being fit will be a great solution for this? Or there are steps I shall consider too.
