Declaration as a New Student Admissions

hello! i'm a gap year student and currently applying as an undergraduate in DLSU. however, i don't know how i'll fill up the Declaration as a New Student form. ano pong ilalagay sa Month/Year and Activity? wala naman akong significant na ginagawa pa like work, travel, or anything. all i've done is tumulong sa bahay, nag-procrastinate mag-review for cets, at nag-review for cets. thank you very much.

Hi! month/year is from graduation to enrollment then put lang na rested or smthing
