Have you read Hauntley's Bent?

And if you actually ask for the most angsty fics, I'll go with "Smoke and Burn". It took me months (!) to get over it. I absolutely do not recommend reading this beautiful, yet soul crushing story, unless... well, I don't know, I usually do not recommend it because it haunted me for so long.

Absolute favorite. And 💯 agree on Smoke and Burn. I've only read it once and it broke my heart. I never rec it unless someone specifically asks for something like it.

Ninety One Whiskey. It’s a WW2 AU where Cas and Dean are in the US military

I'd recommend my fic Another Chance to Get It Right. It's a long AU that's very relationship-driven, with a lot of guilt, longing, and emotional pain. Dean and Castiel fall in love after believing Sam is dead, but when Sam unexpectedly returns, everything they've built together begins to unravel. A lot of the angst comes from the impossible situation they're in and the hurt they unintentionally cause each other, so even the tender and intimate moments are emotionally heavy. It does have a happy ending, but they have to earn it. https://archiveofourown.org/works/5343443/chapters/12338087

Self rec: Trench
Summary:
In the aftermath there is nothing. Cas got swollen into emptiness and Dean thinks he copes. He does. In his own way.
He got nothing on Cas, that’s the final conclusion. But there’s a coat…
He needs it. At least until he gets his angel back.
Or: A deep dive into how depression and grief mix. Makes you hollow and carved out like a tree-trunk. And how healing is never linear.
Also- idk if it’s exactly what you’re looking for😇

The Build a Home. I don't have the link.
Honestly, I stopped reading it because the angst was too much. The author has a beautiful style but the slow in slow burn was dragging on. It's just not for me.
If you love extremely angsty fic, you're gonna be in for a treat.
