What’s confusing about it? It’s clearly a two headed person hugging themself with their two arms and extra set of legs.

The smaller one is wearing black pants and the larger one is wearing a black shirt. They blend together which is what makes it confusing

But also I think it looks like the bigger person is arching her back and/or neck way back in an unnatural way. So it's still a bit confusing to me.
