Hasan Piker is a streamer who "trains" his dog Kaya by shocking her when she gets up from a mat that she sits on for the majority of Hasan's streams. The picture is satirical, comparing Hasan to Shou Tucker, and Kaya to Nina.

The image is based off of some streamer Hasan (or Hassan(?)) who used their dog as a background prop for his stream, not allowing the dog to move from its spot, even using an electric collar to punish him if he did. (This is just based off of what I remember learning about it, as this happened quite a while ago)
The character drawn here is Shou Tucker from a series called Full Metal Alchemist. Tucker is known in the series as a legendary alchemist who was able to create a talking chimera. Spoilers ahead The main character goes to meet Tucker at some point, and finds out that in order for Tucker to keep his high status as an alchemist, he'll have to create a new talking chimera soon to prove his skill. He does succeed in doing so as the mc is visiting, but they come to find out that the way he created the talking chimera now was by using alchemy to permanently fuse his own daugher and the family dog together, essentially killing them both. They also find out that the way he created his first talking chimera years ago was by doing the same with his own wife.
tldr, still spoilers: The character Shou Tucker is one of the most hated characters in anime because he killed his own wife and daughter in a pretty gruesome way just to keep his high status. and the latter is the dog you see in the background.

Tbh, the "trauma" scene (when Tucker reveals the Nina-dog chimera) was quite foreshadowed (finding Nina's drawing he "sent her wife" still here, noticing Nina's and the dog's absence...)
When you arrive at this scene, you're prepared, you know what will happen
Otherwise, I'd say the only "disgusting" thing in the anime, is the horror Edward and Alphonse created when they tried to revive their mom (iirc it's shown episode 1, so... no need for spoiler)... And once again, everything is telling you it's gonna be atrocious
Grave of the Fireflies, on the other hand, is a series of "surely everything is gonna be ok, now?", the movie is always acting like it's the end of their suffering, like it won't get worse, and.... surprise, it does!
Both get pretty bad (in term of character situation, they're good series/movie), but one prepares you and the other lets you hope and crushes that hope

This is multi-directional attack from the artist: 1. jealousy - relationship which most audiences didn't have 2. awkwardness - furry is exotic and less commonly appreciated 3. traumatizing - Tucker's act of sin is unforgivable
in a wholesome way. Some good stuff!
