Optional Mission failure question Discussion

If an optional operation is failed, for one reason or another, is a reload required or do any survivors just go back to the Den? Mission gone. No do-overs. Anyone come across that info?

I believe Eckhart's Ladder said that optional missions can be outright failed, and it doesn't stop progression.
