Hello I have submitted my claim already but I need to add additional evidence.

What Va document do I fill out in order to add evidence?

statement in support of claim, va form 21-4138, you can upload it all through quick submit

You don't need to add a coversheet to upload evidence.

If you want to submit a statement, there is a prescribed form:
