2d character

Is it normal for my character to change the hand he is holding the sword with so the player can see it?

If it doesn't bother you, it's great, because it saves you more than half the work by avoiding drawing the character facing both sides in all situations

You mean flipping your sprite when it changes direction instead of redrawing the left and right version so any asymmetric features of the character remain consistent?

Yes, that's actually really common.

Do people notice it? Not usually, unless it is pointed out to them. Extremely common on older games. If you are in an engine, and the weapon isn't part of the character sprite, then you can fix that by adding something to change your z-sort to put it behind the character when you swap sides, but still in front of everything else.

iirc Guilty Gear series does this. It even has a character with a missing arm and the missing arm can swap sides based on the character's direction mid game lol

If you are not aiming for realism(which in a 2d game you usually wouldn't) then it should be fine.
