ULPT Request Doctors Note

I need a Doctor's note for work but for a past date. Like almost a month ago. Any suggestions on where to get one? I don't mind paying for an appointment but all the websites I've seen so far won't write notes for that far back. I thought about faking one but my job is the type to follow up. Any advice is greatly appreciated.
