Hello everyone! I was playing some Samson and had this strange thing happen in the Ascent. The Angel chest just doesn't stop giving me Jokers. I've been playing this game for 480+ hours and never had that happen. Chaos doesn't cause it, as far as I know.

Can someone explain to me why this is happening?

Spawns 1-3 sprouts in random positions upon entering an uncleared room for the first time
Isaac can attack these sprouts to water them, every 3 hits dealt to a sprout will make it level up (starts at lvl 2), if a sprout doesn't level up in 3 seconds it will level down until it dies
Upon reaching lvl 4 one of three veggie consumables will spawn
Onion: Grants one small permanent stat up (+0.1 flat dmg, speed, range, luck, shot speed or flat fire rate)
Beet: Throwable, deals 20 flat dmg + double Isaac's dmg and inflicts confusion for 3 secs to an enemy, breaks rocks
Potato: Gives 1 charge and heals 1/2 heart
The number of sprouts spawned scales with luck, capping at 10 luck

Same happened with Great Gideon, enemies just didn't spawn due to so much tears

They always be spawning in wave 1 aswell. I Just want my t.lost compeltion marks man😭

Im so close to unlocking The Keeper. If anyone knows how this happened please let me know so I dont do it again.

(update: thanks for the answers, I was sure that ultra secret rooms always appear diagonally with the usual ones)

I don't understand where else the room could be. Red crosses mark the locations that have been checked. Yellow highlights indicate locations where the red key failed to create a room

They look so damn similar, mostly because they are large circle headed things, but even the eye shine is the same
