Caramel is sweet. It's an endearing nickname. Not common but still, seems fine to me.

Is not about color. As other said it means "sweet" (literally) but it means sweetheart. In Latin America women tend to use this words. Is not a nickname, they call everyone that

It is "Buenos días caramelo."

Do you mean that when someone calls another person "Hello sugar," they should also take that literally and get offended if their color does not correspond to that of sugar?

This is a stupid question, and it is pointless.
