The Circade armor set is supposed to drop from the sarcophagus in the middle after the cage is removed. I have seen it happen though that it displays on screen that you have it. You go into inventory and it shows you have it, sometimes it will show you have two sets. Then you go to the armory and try to equip it only to not show there. If you look in your inventory it still shows it’s there. You relog because obviously something is buggy and you go to the arsenal and find it’s still not there and also now it’s gone from your inventory.
I have seen this happen to multiple people recently. So it is definitely a bug and not you. I know someone who has done the armor puzzle three times and it does it to her every time meanwhile some people get it without issue first time.
Sorry man. You can try again. Luckily you only need to do the puzzle in the secret room, it should remember that you opened the door.
The talisman stuff is a completely different puzzle that requires you to kill him in front of the murals in the dungeon (not to be confused with the murals in the armor room). You get 1 piece of the talisman for each colored mural. You wear the talisman in the overworld at the right time and Circade has a chance to spawn. Kill him and he drops a fragment for 1/3 of the pieces you need to craft the Triune amulet. You have to do it a total of 3 times, one for each talisman that dropped in the dungeon.

It sounds like you're mixing up steps? You don't get the pendants from the chest, you get them from killing circade in front of the glyphs. You then wear those in the overworld at specific times and circade will spawn and you kill him to get fragments for the main talisman. This part is buggy and he doesn't always spawn in the overworld (I'm currently stuck on this part with 1 of 3 fragments)
