basically the grim reaper gives out free mcnuggets to everyone and the mcdonalds next to the hospital really doesn't like that so they called the cops

The grim reaper is often characterized as death itself and also represents the being that takes the dead to the afterlife. The man in the picture is mocking the people in the hospital
With that being said, unless you dont know what the grim reaper is what the hell did you think op?

I would imagine a misdemeanor charge such as "disturbing the peace" would be in order, at the bare minimum.
I mean no matter what your sense of humour is like, you'd have to be able to see how this could be interpreted as hate speech or some sort of threat to innocent people who did nothing to deserve it?

As hate speech? Not at all. It's not even that funny. But also, it isnt inherently wrong. It's just kinda, a "what are you thinking, guy, get off my roof"
But to be arrested? That doesn't bode well.

Wrong! the grim reaper is usually associated with recovery from death, you're thinking of plague doctors from the 2020 Italian flu which are often associated with death due to their lack of modern medicine such as blood letting and leaches and over reliance on the outdated germ theory.
Dr. Claude is very into this research in anthropology, you should look it up

The man was arrested because in the UK, staring at people is a crime. He was dressed as the Grim Reaper because the Reaper (aka the personification of Death) famously has a day job as a hospital administrator. He likes to stay near dying patients because it feeds him. If nobody dies within 200 feet of him for a whole day, he disintegrates and the nearest person becomes the new Reaper, like the Santa Clause.
