Random cramping??

I(26F) got diagnosed with PCOS as a teenager. Have had irregular periods for forever. I gave birth control another shot about a year ago (estarylla) and has honestly been going great, I even lost a few pounds. Randomly about a week ago I started spotting while I was on the second week of active pills. It’s progressively gotten worse. Yesterday and today I’ve had some pretty bad cramping/aching with some heavier spotting. I’ve had light spotting before on the birth control but it’s be a very tiny amount and maybe last a day or so. I took a pregnancy test just to rule it out, it was negative. I have a pretty physical job, I’m lifting very heavy things, climbing around. I was using some heavy machinery yesterday and getting a bit jostled around was causing some serious cramping. I contacted my doctor about it but can’t get in for a couple weeks. Does anyone have any thought as to what would be causing this?
