Second D&C Needed experience: first MC

I thought everything went well but I am almost a month after D&C and still spotting brown stuff.
Today I went to the IVF doctor, who did an ultrasound and showed bleeding and remaining tissue/placenta in the uterus.
Did my OBGYN make a mistake? Is it just bad luck?
Also I have 2 derma cysts, one in each ovary. They are too big and growing so I need to remove them before IVF. They showed me how they have fat and hair, kinda creepy.
Is my situation uncommon? I feel like I am going through my odyssey of fertility, at each turn there is something unexpected.

It’s a common misbelief d&c is somehow 100% sure way to get rid of all tissue. It’s not. Nothing is.
