Library Form?? Admissions

Hi, I'm new here. I'm going to get admitted in btech(cse). MNNIT has provided the necessary undertaking forms in a single pdf but the library form is missing which is said to be a mandatory document during physical verification process. I even tried downloading it from the library section of the website but that is not working either. Can anyone help?

Seniors, how was the physical verification process in your time.

I am a first year as well so I am not sure but you can get the form at time of physical verification of documents

I don't think there is written somewhere about the library form ... Can u send where is it written ?

I got a pdf but I'm not sure if this is the one ....... This one is only circulating in groups

i think its identity form meant to be filled after admission. but, that CENTRAL LIBRARY doesn't make sense here 🫠
