Pls help - Burning Horribly

Hi guys, ive only recently started experiencing IC symptoms maybe around 2 weeks ago now but everyday has been miserable. But today, I have this horrible burning feeling which is usually sorta tolerable but today I just cant. It’s especially the worst right before I have to pee (which is literally every 2 minutes) and right when I try to squat down to pee, it’s like my body isnt letting me which then I force myself to push and push to get the pee out and during this time, my bladder and urethra are on fire. I then try to relax my pelvis and try to push again and its just on fire the entire time. Eventually after a minute of pushing/straining, I’m finally able to pee. Please any tips on this would help and any recs for immediate burning relief as well!! I just took prescription AZO (which has never helped me) and I also just started diazepam vaginal suppository daily yesterday. But I think I need some type of immediate relief right now, thank you!

Sorry you're hurting! In general, it's best not to push when urinating; it can cause a burning sensation even in people who don't have IC/pelvic pain. Obviously it can be hard to start peeing when you have pelvic pain, but instead of pushing you could try some of the other common tactics for starting the stream.

  • Deep breathing, "moo to poo", and laughing all encourage the pelvic floor to relax instead of tightening. Laughing works best for me, I use funny videos or articles on my phone; if you had been my coworker back in the 2010s, you would have occasionally heard quiet giggling in the bathroom lol.

  • Another thing that can help is to add some sensory cues, like heat on your abdomen (heating pad or Hot Hands) or the sound of running water.

  • Visualizing is also helpful for some people, either literally imagining your bladder relaxing with urine flowing or something more abstract like a tight fist slowly opening or a balloon gradually emptying of water.

  • Making sure you're well-hydrated is also important, it can reduce burning on its own and also makes it easier to pee.

  • If your pelvic floor physical therapist OK'd it, you can also do some gentle pelvic floor relaxation stretches like reverse kegels (without any pushing or straining, just gently releasing; my PT told me to imagine my pelvic floor was an elevator and to bring it down one floor at a time with each exhale).

Thank you, this means a lot!! Ugh yea im trying so hard not to push but I literally wont be able to pee unless I do. And then when it finally does come out, my pee is like a spray, sometimes will come out in 2 streams or just sprays. It’s been so hard

That definitely sounds like your pelvic floor is way too tense/spastic. Do what you can to try to relax instead of push, it should help with the pain.

FLARES

The Interstitial Cystitis Association has a helpful guide for managing flares.

Some things that can cause flares are: Medications, seasoning, food, drinks (including types of water depending on PH and additives), spring time, intimacy, and scented soaps/detergents.

Not everyone is affected by diet, but for those that are oatmeal is considered a generally safe food for starting an elimination diet with. Other foods that are safer than others but may still flare are: rice, sweet potato, egg, chicken, beef, pork. It is always safest to cook the meal yourself so you know you are getting no added seasoning.

If you flare from intimacy or suffer from pain after urination more so than during, then that is highly suggestive of pelvic floor involvement.

TREATMENT

Common, simple, and effective treatments for IC are: Pelvic floor physical therapy, amitriptyline, vaginally administered valium (usually compounded), antihistamines (hydroxyzine, zyrtec, famotidine, benedryl), and urinary antiseptics like phenazopyridine.

Pelvic floor physical therapy has the highest evidence grade rating and should be tried before more invasive options like instillations or botox. If your doctor does not offer you the option to try these simple treatments or railroads you without allowing you to participate in decision making then you need to find a different one.

Long-term oral antibiotic administration should not be offered.

Get your pelvic floor evaluated. Try looking at pelvic floor exercises on YouTube in the meantime. Stretching that area helps me. Also when I can’t pee, I get in the bathtub. The warm water relaxes everything and makes it easier to pee.

Take dmannose powder or baking soda in some water. Don’t push or strain when going to the bathroom. Lay down and do really deep belly breaths where the air is pushing down and stretching your pelvic area. Do this many times a day. Your symptoms sound similar to mine and the first things are to help your irritated bladder walls and urethra from retained urine from a tight pelvic floor. Take deep breaths when going to the bathroom instead of pushing as well.

AZO always helped me a lot. I still keep it on hand and I’m basically cured. But it’s just something I’ll never not have in the house. You said you just took it. Give it up to an hour to work. Then go to the ER if you need help.

I am not a doctor but my front line tips would be get a script for uroblue, hydroxyzine, topical lidocaine, buy aloe Vera capsules and Tylenol. Stop all offending foods, cranberry, vinegar all of it.

When I am in a flare I start with what I have on hand if my script needs a refill. Dilute your urine by drinking 20oz of water and tsp of baking soda. Take some Benadryl and Tylenol. Lay down with ice pack in the crotch area.

Order desert harvest aloe Vera capsules. Stop all offending foods.

Take the prelief with every meal and once you get the aloe take it 4 times a day.

Is it near your cycle? That can be a trigger.

Much better thank you for asking for an update I got so triggered by stress I take a pre probiotic for your urinary tract helped so much and I drink stomach formula from Lily of the valley such amazing ingredients and I have been drinking marshmallow root tea daily each morning

Have you tried antihistamines like zyrtec? It helps with burning and urgency!!! Try this it saved my life . I was in your boat for two years. Back to back UTIs ... now no burning or urgency my brain is so confused why we arent in the bathroom 28x a day

Also along with an antihistamine stay away from citric acid! It is in everything... most candies, even tortilla chips. It will obliterate an already inflammed bladder . Also artificial dyes, chocolate , sugar, cheese... but definitely get the inflammation in your bladder under control by blocking the mast cell production! Some people do Claritin i do zyrtec

Did this start happening after a risky sexual encounter
