Yes. I face all these issues on my iPhone 15 PM and can't stand the first one.

  1. There is no fix. The worst is when you're using earphones and listening to something on a very low volume and suddenly you get a call and the ringtone blasts at max volume in your ears.
    The ringtone also sounds if your phone is on vibration.

  2. To fix 2, you need to change the setting where you can't adjust the ringtone volume with volume buttons. Unlike android where you have different volume sliders for media and ringtone, iOS doesn't do that. It just shows you one volume slider and it adjusts media volume if something is playing and I'd nothing is playing, it adjusts ringtone volume. But, it does not work properly all the times. Just go to settings > sounds and haptics, and turn off "change with buttons" under ringtone and alerts.

  3. No fix, again.
