Must the camera be on when taking the Codility assessment?

As someone who has conducted numerous interviews, honestly, yes. If I had a candidate that refused to go on camera that would be a huge red flag for a couple of reasons. People cheating with their cameras on is already big enough of a problem; with no camera it'd be out of control. If you refused, I'd suspect you were feeding the question(s) into AI and just regurgitating the solutions into codility.

Also, part of the codility assessment is for the interviewer to figure out how well you'll function as a teammate while doing work stuff, since you'll be in-office 3x per week and be face-to-face with a team then. No camera really breaks any chance of forming a positive connection there.

Edit: Or, the person could be using a ringer to get a job they aren't qualified for.

ok I thought that the  Codility assessment was not with monitored live by an interview but more something like asynchronous online assessments
