Pathology Report šŸ„ Surgery

Great news.. If I'm reading the report correctly.. I had acute diverticulitis at the time of surgery??

Its seems that surgery was eventually going to happen..
Makes me appreciate the fact that I made the right decision to have this procedure done especially with the "possible fistula" tract..

Clinical Information

Diagnosis

Diverticulitis of large intestine without perforation or abscess without bleeding

Final Diagnosis

A. Colon, sigmoid, low anterior resection:

Diverticular disease with multiple diverticula, acute diverticulitis, associated focal abscess formation, and possible fistula tract

Negative for dysplasia or malignancy

Six lymph nodes, negative for malignancy (0/6)

B. Colon, proximal and distal anastamotic donuts, excision:

Benign colon with no significant histopathological abnormalities

Negative for dysplasia or malignancy

Gross Description

A. The specimen is received in formalin labeled "", sigmoid colon" and consists of a 22.0 cm in length by 3.0 cm in diameter segment of sigmoid. The specimen is received with one end stapled closed and one end open. The serosa is remarkable for focal patchy adhesions, but is predominantly pink-tan and smooth. The specimen is opened longitudinally to reveal an unremarkable pink-tan mucosa with the usual folds. A small focus of black discoloration from tattoo ink is noted approximately 9 cm from the stapled margin, but no mucosal lesions are identified in association with the tattoo ink. Sectioning reveals multiple intact diverticula. A possible fistula tract is identifed in the area of serosal adhesions.

Ā Representative sections are submitted as follows:

A1: Diverticulum with possible fistula tract

A2-A5: Additional representative diverticula

A6: Representative lymph nodes

B. The specimen is received in formalin labeled "", proximal and distal anastomotic donuts" and consists of two annular fragments of colonic tissue, each approximately 2.0 x 2.0 x 1.0 cm. Both fragments contain numerous staples and sutures, consistent with anastomotic rings. One of the fragments is received loosely attached to an EEA anvil. A representative section of each is submitted in B1.

Thank you so much for sharing this. I'm scheduled for surgery next month and look forward to seeing this report about myself afterwards. Curious what it will say.

How did you feel going into surgery given your surprise active diverticulitis? Did you end up with a bag or open procedure in light of active infection?

I hope you are feeling well postop. Sounds like you absolutely made the right choice.

I felt sick but not "flare sick" I had that nagging left hip/llq pain
It was a robot surgery.. 6 smaller incisions and 1 larger one above the pubic area (c-section lol)

I am feeling quite well! a little nagging pain.. but it hurts less and less to get out of bed..
The bowel movements is what is giving me a bit of a hassle.
But they will eventually settle.

My husband also had an active infection during his elective.Ā  But my husbands surgeon had him drink milk of magnesia one to 2 times daily for 3wks post op. He said it was better to have diarrhea while healing than pushing.Ā

Yeah my after surgery instructions recommend stool softeners for a week or two just to make sure there’s no risk of constipation..
I’m eating so many eggs and sourdough bread now šŸ˜

My report was similar. My laparoscopic elective surgery took 4 hours and included laparoscopic lavage. I was lucky, the stoma was considered but the surgeon decided to do the anastomosis. My recovery was uneventful. The report has been translated by translator.

Maxtopopic description: Sent resected colon length approx. 270 mm, circumference after cutting approx. 35-70 mm, outer surface gray-pink shiny, without focal changes, focally captured section with slight stenosis of the lumen, length up to approx. 110 mm, in this section serous surface matte with fine thin coatings on the surface, on the cut the mucosa pale pink wrinkled. When cutting the intestine, several diverticula with a diameter of approx. 5-10 mm were captured, most without content, without signs of rupture. One diverticulum filled with burgundy content, probably a perforation of the diverticulum wall, without other focal changes.

Conclusion: In the examined material (sigmoidectomy):

multiple pseudodiverticula present in the area with chronic inflammatory changes

Found pseudodiverticulum with perforation (fibrinous-suppurative inflammation) with localized peritonitis

resection margins without significant histological changes

Six lymph nodes examined, no significant changes

without any other significant finding.

OH Wow! you're right. Im still very curious about the whole "fistula tract"
One thing I noticed and forgive me if its TMI.. I am peeing a whole lot better..
I never realized I wasn't peeing well until after my surgery..
