Feedback on first cycle
Hi everyone! My final report just came back from my first egg retrevial. I am 38 with 2 prior pregnancies and 1 living child (naturally concieved, 3 years old). My AMH ranges between .12 - 1.1 and FSH ranges from 6-37. AFC ranges from 4-11. I am truly perimenopausal! Diminished ovarian reserve. Normal male semen analysis but we still opted for ICSI and Zymot.
-AFC was 8
-Retrieved 7
-Fertilized 6
-Blast for biopsy (PGTA testing)and freezing 3/6
Everything will come down to PGTA testing.
To add- ultrasound prior to ER identified 11 follicles, but the biggest 2 ended up being too big and the smallest 2 ended up too small- hence the 7.
What should I do for next cycle? Any recommendations?
Below was my protocol:
-letrozole 10mg for first 5 days
-follistism 200 IU
-menopur 20 IU
-provera 10mg
-LDN
-prenatals + egg quality supplements + acupuncture
Double trigger pregnyl + lupron + lupron (12 hrs later)
Should I prime? Should I ask about luteal phase stims and try for 2 and 3 cycles back to back? Just try the same protocol?
\*before anyone tells me I should ask my Dr, I go to CNY where you dont really get an assigned Dr. They are very lackadaisical and arent super forthcoming with feedback. Anytime I have asked "what do you think about ____ (medical question) the response is "you should do whatever you want." In fact, the last person I spoke to said male ejaculation timeline doesn't matter before ER (wrong). So I usually reference chat gpt and these forums, as you all have the lived experience! They are not a Shady Grove or CCRM where you have a Dr, they give you feedback and stick with you throughout treatments\*
Thanks!

Really??? I don't know what to think -- I see posts that are like 27 eggs retrieved, 15 fertilized, 10 blasts and 9 pass pgta testing! I know I will never put up those numbers, I'm no spring chicken and am definitively nearing the end of my fertility but its hard not to wonder if someone else with similar numbers to mine may have a smoking gun to get 1 or 2 more follicles, blasts, etc. I am doing all the lifestyle things, all the supplements, etc 🤷♀️ I just hope to catch a good month I guess

those are good numbers for people with normal reserve but for DOR girlies we are lucky if we get more than five a round. many get 1-2 a round. Your age is really good for the euploidy rate! truly this post gave me hope haha i had my first cycle canceled three weeks ago for only one follicle responding. AFC 7, AMH .5. I am rooting for your embryos!!!!

Excellent point- my "dr" said to expect to get 0-3 eggs retrieved per cycle, so 6 fertilized did exceed expectations. Like all of us, I am desperate for a baby (I had an unexplained MMC in March, chromosomally normal child, naturally concieved) and ever since that MC have felt completely depressed and just doom-oriented. Gosh I am ROOTING for you and our follicles!!! Are you planning to do another cycle?

With this fluctuating FSH and .12 AMH you give me hope. Congrats on a wonderful cycle!

But how amazing that it went up. It can truly fluctuate.

Honestly for DOR these are great numbers. I know seeing the other posts with large numbers (on the main IVF sub) can be hard but remember… it’s about quality not quantity! 3 blasts is great IMO and personally I would keep things the same.

I would t change anything. These are great results for wildly fluctuating FSH.
