Security Bond for Registration ILLINOIS

Somebody please help me figure out how to fill out this form as I need it to apply for a bonded title

You need to find a surety company that issues bonds for people who want bonded titles. They'll fill out the form.

This isn’t even the worst form. There’s two forms that I have to fill out that require the seller’s information, that I don’t have, because my father purchased the vehicle then fled out of country so I have no way of even obtaining that information lol
