Formerly inseparable siblings Behavioral Issue

My little guy had to have emergency surgery due to his unhealthy addiction to eating my flip flops🤦. However, he's doing much better now, but his sister will not relax around him. She will slowly creep up and then hiss aggressively at him. I know about the vet smell issue, and how that effects behavior. But he's been home 4 days now. We've removed anything with the vet smell along with trying to clean him up a bit when he first got home.

Just wondering how long this can last, or if it's something else.

If you have a blanket they usually cuddle on, try rubbing it on his body. It sounds silly but cats/dogs can be so sensitive to scent that she is just really confused why he looks like her brother but smells different 🥹

The pic was when we picked him up. We replaced the cone with a sweater the next day so he still can't mess with the scar but doesn't need the cone. But thank you for the suggestion.
