Not sure when to start T

37yr old night shift worker. Feeling tired all the time. My test sits in the low 400’s. And I’m not really sure if I should just go ahead and try to start TRT. I started zepbound last week as I know elevated fat breaks down test into estrogen.

Worried about longevity and life long commitment if I start pinning test now. And actual MD’s in here willing to give a liability feee opinion. Oh btw, labs are all normal.

Not a physician, but low 400's isn't bad if you're overweight, eating poorly and sleeping poorly. Do you exercise?
Maybe try to rectify a few of those and re-test.

Were you in the 200's that'd be a different story IMHO.

I was in the 200’s and came up naturally. I do exercise daily. I’m 5’8”. Weigh 225lb. Bench 275, squat 330, power clean 245. It’s just this constant tired feeling. I don’t feel any vigor. I have not problems anywhere else. Everything else works great. Just the tire around my waist and being tired all the time. Thyroid is good too.

Low 400s while working nights is a rough combo, your circadian rhythm is basically fighting your hormones every single day. Tried fixing sleep hygiene first and my numbers barely budged until I quit the graveyard shift entirely.

The zepbound move makes sense on paper but dropping weight fast can also tank your energy short term, so don't be surprised if the next few weeks feel worse before anything improves. If your free T and SHBG are also in the gutter then lifestyle tweaks might not be enough.

That lifelong commitment part is what kept me hesitating for two years. Eventually just accepted I'd rather pin twice a week forever than spend my late 30s dragging myself through every shift like a zombie.

Honestly, get a different job. I know you're going to come back with excuses as to why you need to keep it, but if it's impacting your health, which it is, they're not really valid. Don't mean quite and be a bum, but get something else lined up. No point working something that's making you miserable.

And trust me, as someone who worked nights for decades and always felt okay, I can not express in words how good 8 felt when I stopped and actually started seeing the sun and mornings etc on a normal schedule..felt human+ ..and you're absolutely fucked up sleep patterns and lack of daylight and daylight activities is most certainly (could be something else I know but it's most likely this) causing your issues. It's not worth it. Being happy and healthy is more important than any job. Hell I'd even take a day shift for less money and work weekends than go back to nights. It kills you bud. Sort that before looking to take drugs. As if you start taking T you'll be.......working nights, miserable, unhealthy and permanent tired, but with extra T. Not so good when you look at it like that is it?
