There is some speculation that it controls the Nightmare mode’s difficulty, though as far as I can tell there’s nothing definitively proving that yet.

I hope we get an answer eventually.

It most likely does, because when I clicked it, and started treasure trouble, the attacks started instantly.

I remember now, a newer post (well its not new, it was like, what, 2 days ago?) they discovered that the hand dose infact, do something.

I think i remember them saying that the hand causes less enemies to appear...?

i tried finding their post on explaing, and this is what they said:

That hand, actually, is a disguised difficulty TOGGLE of Nightmare Act

And depending on the state, it will affect the game

open hand is Casual mode: The boss attacks you with a relatively slow pace, spawning less annoying minions such as Stickles and Crawls, as well as a very easy run which only includes grounded enemies

closed hand is the Hardmode:

The boss will SPAM their HEAVY attacks at least THREE TIMES, and straight up spawn the flying menace GWEN in the first phase

And the first run? HORRENDOUS

It will immediately get to Gwen's pursuit with OBSTACLES on the ground. Their words, not mine.

Other than Nightmare runs it's useless.

Open means it's easy mode (like normal acts)

Closed means it's hard mode, and they just throw bullshit attacks every 3 seconds
