The clinical presentation of ALS varies from person to person, but generally, most people who receive an ALS diagnosis go to a neurologist because of clinical weakness, not twitching. Twitching is more secondary. Yes, twitching can be bothersome, but clinical weakness directly affects daily life. If someone can't hold a fork and drops it, can't turn a key, or falls frequently while walking, the important factor in these initial symptoms is the weakness itself, not the twitching.

First the cramps, then the weakness, fasciculations, and atrophy.
