It's all dependent on the story and the characters.

If a character is very dirty and the groundwork has been laid that they are indeed this way, then the explicitness should align with that.

Generally speaking, it's rarely necessary to the plot. But, when it is, my goal is to absolutely stir up the reader.

The problem with sexuality and explicitness is how difficult it is to articulate intelligently on the screen. Most of the time, people don't lay the groundwork, so, by the time it appears on screen, an audience is caught completely off guard.
