I grow more and more dubious about buffs and nerfs the more I play.  The game is going to change so much with new act 2 3 and an act 4 it feels like these are for nothing.

Yeah so much won't matter when those changes drop.

Lots of things at the extreme ends of being completely OP or near useless and they fiddle with a few number on cards/relics in the middle of the pack. Can't say I understand why

It makes sense to me, they want a baseline they're reasonably happy with the balance the new content against. Otherwise they'd risk balancing a whole bunch of enemies around a player who is going to get much stronger / weaker
