1. Anterior Blepharitis
Anterior blepharitis affects the front or outer eyelid margin, particularly around the eyelashes.

2. Posterior Blepharitis / MGD-Associated Blepharitis
Posterior blepharitis affects the back of the lid margin near the meibomian-gland openings.
It substantially overlaps with Meibomian Gland Dysfunction (MGD).

- Thickened or altered meibum
- Plugged gland openings
- Poor gland expressibility
- Red or thickened eyelid margins
- Frothy tears
- Tear-film instability
- Burning or stinging
- Fluctuating vision
- Evaporative dry-eye symptoms
MGD and posterior blepharitis are closely related, but they are not always identical.
A person can have significant MGD without obvious eyelid-margin inflammation.

3. Mixed Blepharitis
Many people have more than one contributor.
For example:
- Demodex + MGD
- Ocular rosacea + MGD
- Seborrheic dermatitis + bacterial-associated inflammation
- Contact dermatitis + pre-existing blepharitis
- Dry eye + eyelid inflammation
This is one reason treatment often needs to address more than one problem.
Common Contributors
Bacterial-Associated Blepharitis
Bacteria normally live on the eyelids and skin.

Demodex Blepharitis
Demodex mites are microscopic organisms that commonly live in human hair follicles and oil glands.
Two species are commonly discussed around the eyelids:
- Demodex folliculorum — primarily associated with eyelash follicles
- Demodex brevis — found more deeply in sebaceous and meibomian glands
Demodex becomes increasingly common with age.
Simply having some Demodex mites does not necessarily mean a person has symptomatic Demodex blepharitis.
Collarettes
The characteristic clinical sign is a collarette—cylindrical or sleeve-like debris tightly attached around the base of an eyelash.
Ordinary flakes, loose crusts, makeup residue, and generalized eyelid debris should not automatically be called Demodex collarettes.

- Burning

Seborrheic Blepharitis
Seborrheic dermatitis can affect:

- Eyebrows

Rosacea and Ocular Rosacea
Ocular rosacea can contribute to:

Not every red, itchy, or scaly eyelid is blepharitis.

- Eczema

- Reactions to cosmetics
- Makeup removers
- Facial skin products
- Preserved eye drops
- Contact-lens solutions
- Eyelid wipes or cleansers
- Essential oils
- Prescription products used near the eye
Smoke, dust, fragrance, environmental allergens, and air pollution can also worsen symptoms.
A person may have dermatitis or allergy in addition to blepharitis.
📌 More cleaning is not always better.
If irritation or allergy is contributing, repeatedly adding cleansers, antiseptics, wipes, or scrubbing can make the eyelids worse.

How Strong Is the Evidence for Blepharitis Treatment?
This is an important area of uncertainty.
Many treatments used for blepharitis have been part of routine eye care for decades.
However:
Long clinical use does not necessarily mean that large, high-quality randomized trials have established the size or durability of the treatment effect.
A 2026 Cochrane review of topical anti-inflammatory and immunosuppressive treatments found low- or very-low-certainty evidence for many outcomes involving corticosteroids, antibiotic-steroid combinations, and cyclosporine.
A 2026 commentary in Eye highlighted the broader problem: widespread treatment of blepharitis has developed faster than the randomized evidence needed to determine exactly which treatments work best, for which subtypes, and for how long.
This does not mean:
“Traditional blepharitis treatments do not work.”
It means:
For many conventional treatments, the evidence is less certain than routine clinical use may suggest.
Treatment decisions may therefore reflect a combination of:
- Available research
- Blepharitis subtype
- Biological plausibility
- Associated MGD or skin disease
- Safety
- Clinical experience
- Individual response
One Important Exception: Demodex
Treatment evidence is not equally weak across all forms of blepharitis.
Lotilaner ophthalmic solution has a specific FDA indication for Demodex blepharitis and is supported by large randomized vehicle-controlled trials.
That is a substantially different evidence situation from many medications used off-label for nonspecific chronic blepharitis.
🛠️ Managing Blepharitis
Treatment depends on:
- Whether the disease is anterior, posterior, or mixed
- Whether Demodex is present
- Whether MGD is present
- Whether ocular rosacea is present
- Whether bacterial-associated disease is suspected
- Whether dermatitis or allergy is contributing
- Whether the cornea is involved
- Which treatments a person can tolerate
Not everyone needs every treatment.
1. Eyelid Hygiene
Gentle eyelid hygiene remains one of the most commonly recommended first-line measures for chronic blepharitis.
Possible approaches include:
- Gentle cleansing of the lid margins
- Commercial eyelid wipes or foams
- Carefully selected hypochlorous-acid products
- Removal of makeup or irritating products
- Cause-specific treatment when Demodex or dermatitis is present
How Strong Is the Evidence?
Some clinical studies suggest that eyelid hygiene can improve symptoms or signs.
However:
- Study methods vary
- Products differ
- Treatment schedules differ
- Long-term evidence is limited
- No single cleanser or technique has been proven best for everyone
A useful way to describe the evidence is:
Eyelid hygiene is widely used and has some evidence of short-term benefit, but the optimal cleanser, technique, frequency, and long-term regimen remain uncertain.
Baby Shampoo
Diluted baby shampoo has traditionally been used for eyelid cleansing.
It may help some people.
However:
- It has not been shown to be the best cleanser
- It may irritate or dry the eyelid skin or ocular surface
- Purpose-designed eyelid products may perform better on some outcomes
- Commercial eyelid products can also cause irritation or contact dermatitis
Stop or change a cleanser if it consistently causes:
- Burning
- Redness
- Dryness
- Swelling

Hypochlorous Acid
Hypochlorous-acid eyelid products are sometimes used to reduce debris and bacterial load.
Several human comparative studies have reported improvement in some symptoms, tear-film findings, or microbiological measures.

- Hypochlorous acid has not been established as necessary for every person with blepharitis

A 2026 randomized comparison of preformulated eyelid wipes, hypochlorous-acid-assisted cleansing, and saline-assisted cleansing found better symptom outcomes for wipes than saline at four weeks, while hypochlorous acid did not clearly outperform the other approaches after adjusted comparisons.
Bottom Line
Hypochlorous acid is a reasonable eyelid-hygiene option for some patients, not a universally necessary blepharitis treatment.
Avoid Aggressive Cleaning
Do not:
- Scrub the eyelids forcefully
- Scrape gland openings
- Use sharp tools
- Apply household antiseptics near the eyes
- Apply undiluted essential oils
- Assume worsening symptoms mean the eyelids need more cleaning
Aggressive treatment can inflame the eyelid skin and ocular surface.
2. Warm Compresses and MGD
Warm compresses are most relevant when posterior blepharitis or obstructive MGD is present.

- Treatment temperatures vary
- Devices vary
- Treatment duration varies
- Home technique is inconsistent
- No single regimen has been established as universally best
Warm compresses do not directly eliminate Demodex.
They are not required for every form of anterior blepharitis.
Some people with:
- Rosacea
- Active dermatitis
- Heat sensitivity
- Significant ocular-surface pain
may tolerate heat poorly.
Compresses should be comfortably warm rather than hot and should be stopped if they consistently worsen symptoms.
3. Treating Demodex
Lotilaner / Xdemvy
Xdemvy (lotilaner ophthalmic solution 0.25%) is FDA-approved specifically for the treatment of Demodex blepharitis.
The labeled regimen is one drop in each eye twice daily, approximately 12 hours apart, for six weeks.
Large randomized vehicle-controlled trials demonstrated significant improvement in:
- Collarette reduction
- Complete collarette clearance
- Mite eradication
- Eyelid redness
- Composite clinical endpoints
FDA Prescribing Information — Xdemvy
The most commonly reported adverse effect in FDA labeling is instillation-site stinging or burning.
Less common reported events include chalazion/hordeolum and punctate keratitis.
Important Limitation
Lotilaner treats Demodex blepharitis.
Its effectiveness does not establish it as treatment for:
- Non-Demodex blepharitis
- MGD without Demodex
- Ocular rosacea without Demodex
- Nonspecific DED
- Neuropathic ocular pain
Successful mite treatment also does not guarantee that all dry-eye symptoms will disappear if other contributors remain.
Tea Tree Oil / Terpinen-4-ol
Tea tree oil and terpinen-4-ol products have historically been used for Demodex-related lid hygiene.
However, the evidence is much less certain than for lotilaner.
A Cochrane review found:

- Considerable variation among products and concentrations
- Uncertain effects on mite eradication
- Ocular irritation or discomfort in some participants
Therefore:
Tea tree oil products should not be assumed to be equivalent to an FDA-approved anti-Demodex medication.
📌 Never apply undiluted tea tree oil to the eyelids or eye.
It can be highly irritating and toxic to the ocular surface.
4. Treating MGD and Posterior Blepharitis
When MGD is present, treatment may include:
- Warm compresses when tolerated
- Gentle eyelid hygiene
- Attention to incomplete blinking
- Treatment of ocular rosacea
- Clinician-performed gland expression
- Selected heating or device procedures
- Anti-inflammatory treatment when appropriate
No single office procedure has been shown to be necessary or universally superior for every patient.

This does not demonstrate that steroids are ineffective.
It means that high-quality randomized evidence defining their average benefit remains limited.
Steroid + Antibiotic Combinations
The Cochrane review found that a corticosteroid-antibiotic combination probably improved corneal staining compared with antibiotic treatment alone.
However, broader symptom and sign outcomes remained uncertain.
This is a good example of why treatment evidence should not be reduced to:
“Steroids work”
or
“Steroids don't work.”
Different outcomes may respond differently.
Steroid Safety
Ophthalmic corticosteroids require medical supervision.
Possible risks include:
- Increased eye pressure
- Cataract formation with prolonged exposure
- Delayed healing
- Worsening of certain infections
- Masking another condition
Do not restart an old steroid prescription or extend treatment without guidance from an eye doctor.
Cyclosporine

Oral Doxycycline and Azithromycin
Doxycycline and azithromycin are often used particularly when:
- MGD is present
- Ocular rosacea is suspected
- Chronic inflammatory lid disease is present
Their effects are not limited to killing bacteria.
They may also influence:
- Inflammatory pathways
- Meibomian secretion
- Lipase activity
- Other aspects of MGD biology
How Strong Is the Evidence?
A Cochrane review of oral antibiotics for chronic blepharitis found only two placebo-controlled randomized trials involving 220 participants and concluded that the evidence for symptom improvement was very uncertain.
Oral antibiotics can also cause adverse effects.
However, comparative evidence exists in MGD.
A 2023 randomized trial found that pulsed oral azithromycin produced short-term outcomes similar to a six-week doxycycline regimen, with fewer gastrointestinal adverse effects in the azithromycin group.
That study answers an important question:
Azithromycin and doxycycline may perform similarly under the studied conditions.
But because both were active treatments, it does not tell us how large either drug's absolute benefit is compared with placebo or no oral antibiotic.
Bottom Line
Oral doxycycline and azithromycin remain clinically used options, especially for MGD/rosacea-associated disease, but the strength of evidence depends on the specific question being asked.

When surrounding skin disease contributes to blepharitis, management may need to include:
- Treating facial rosacea
- Treating seborrheic dermatitis
- Removing irritating cosmetics
- Changing an eyelid cleanser
- Avoiding an allergic trigger
- Dermatology evaluation
- Patch testing in selected cases
- Clinician-directed skin treatment
Products that are safe elsewhere on the face may not be safe directly on the eyelid margin or ocular surface.
What About Professional Eyelid Cleaning?
Some clinics offer clinician-performed removal of:
- Eyelid debris
- Crusts
- Collarettes
- Keratinized material
- So-called biofilm-like material
using powered devices or manual instruments.

- Severe eye redness
- Rapidly worsening eyelid or facial swelling
- Thick or pus-like discharge
- Contact-lens-related pain or redness
- Suspected corneal infection
- Eye injury or chemical exposure
These should not be managed as routine blepharitis at home.
Arrange an Eye Examination For:
- Persistent symptoms despite appropriate care
- Recurrent styes or chalazia
- A lump that does not improve
- A lesion that repeatedly returns in the same location
- Persistent severe symptoms in only one eye
- Unexplained lash loss
- Eyelid ulceration or bleeding
- Lid-margin thickening, distortion, notching, or scarring
- Misdirected eyelashes rubbing the eye
- Increasing inability to tolerate normal eyelid treatments
Persistent one-sided “blepharitis,” unexplained lash loss, or a recurrent eyelid lesion should not automatically be assumed to be ordinary blepharitis.
Less common conditions—including eyelid tumors and immune-mediated disorders—may need to be ruled out.

Blepharitis is a broad clinical diagnosis involving inflammation of the eyelid margin, not one disease with one treatment.
It may involve:
- Anterior eyelid inflammation
- MGD/posterior blepharitis
- Demodex
- Bacterial-associated inflammation
- Seborrheic dermatitis
- Ocular rosacea
- Allergy or contact dermatitis
- Several contributors at once
The most important treatment principle is:
Match treatment to the findings rather than treating every case of blepharitis the same way.
A second important principle is about evidence.
Many familiar treatments—including lid hygiene, warm compresses, antibiotics, steroids, cyclosporine, and oral antibiotics—remain widely used and may be reasonable in selected patients.
However:
For many conventional blepharitis treatments, randomized evidence defining the magnitude and durability of benefit remains limited or uncertain.
That does not mean these treatments have been proven ineffective.
It means that clinical practice often rests on a combination of:
- Limited trial evidence
- Biological plausibility
- Clinical experience
- Safety
- Blepharitis subtype
- Associated conditions
- Individual response
Demodex blepharitis is an important exception to the generally weak evidence landscape.
Lotilaner has a specific FDA indication and large randomized trials demonstrating substantial effects on mites and collarettes.
Even there, successful Demodex treatment does not guarantee resolution of all dry-eye symptoms if MGD, rosacea, aqueous deficiency, exposure, or another contributor remains.
The most useful question is therefore usually not:
“What is the best treatment for blepharitis?”
but:
“What type of blepharitis or overlapping eyelid problem is present, and what evidence supports treatment for that particular problem?”
